What Is Breast Implant Illness (BII)? Breast Implant Illness (BII) is a collection of symptoms that some people experience after breast augmentation or reconstruction. These symptoms can include joint and muscle pain, chronic fatigue, cognitive difficulties (such as memory and concentration problems), rashes, hair loss, and gastrointestinal issues, among others. BII can develop in individuals with any type of breast implant—whether they are silicone gel-filled, saline-filled, smooth surface, or textured surface implants. Furthermore, BII can occur regardless of whether the implants have ruptured or remain intact. The underlying cause of BII is not fully understood, but it is believed that the symptoms may be due to an autoimmune or inflammatory reaction to the implant materials. BII isn’t an official medical diagnosis yet, but the impact is very real for those experiencing it. 🩺 It can happen with any type of implant and can occur whether the implant is intact or ruptured. Ophthopedia Update:Ab Interno Ahmed Glaucoma Valve Plugs In An Eye With Persistent Hypotony: Purpose: This case report aims to describe a new method for increasing intraocular pressure (IOP) in patients with acute hypotony resulting from uveitis flare-up and preexisting glaucoma drainage device. The temporary glaucoma tube plug method described is effective and safe. Methods: This case report presents a 47-year-old female patient with a history of chronic panuveitis and secondary glaucoma, who had two previously implanted Ahmed glaucoma valves. The patient developed panuveitis flare-up and persistent hypotony. A novel method of ab interno plugging of the glaucoma tubes using 2-0 prolene suture plugs was performed. Following treatment, the IOP increased successfully and remained within the normal range. Conclusion: The temporary ab interno glaucoma tube plug method effectively increased IOP in a patient with two pre-implanted Ahmed glaucoma valves with persistent low IOP due to uveitis. Shoulder capsular distension hack: I had a recent patient who I performed a hydraulic capsular distension under interscalene nerve block on for adhesive capsulitis. The joint took about 35 ccs of fluid. I stop injecting when there is either visible swelling or distension of the capsule under ultrasound or the syringe meets enough resistance to back flow. Once you feel more resistance to injection, inject 1-2ccs at a time and wait a few seconds to see if fluid back flows into the syringe. If there is back flow, you know the joint is reached its max capacity before failure. I do not like the method of rupturing the capsule as I don’t want to create instability and based on this study: Updates on the treatment of adhesive capsulitis with hydraulic distension - PMC (nih.gov). I prefer Platelet lysate and nangogram steroids to mg steroids to reduce inflammation without the steroid negatives. Others may use PRP. Despite nerve block, the patient was having more discomfort than normal when stretching in flexion and extension. It was clear his anterior musculature was super tight, so I decided to perform trigger point injections with lidocaine/ropivacaine into the pecs, subscap, lat and teres major. This helped relax those internal rotators and allowed me to then get near full ROM with gentle manipulation. FYI here is a review of PRP for AC as there are too many studies to list.
