During my morning phone appointment , a patient described a new rash. I transitioned the call to a video visit and guided her through the process of connecting via her phone. Once connected, I was able to see the rash and subsequently revised my initial diagnosis. Her A1C level was 12, prompting me to prescribe a Continuous Glucose Monitor (CGM). Great to see this highlighted, so what about treatment for high blood pressure in #pregnancy? As you might have guessed, there is very little high-quality data to inform treatment of high blood pressure in pregnancy. We use drugs that are assumed safe, but we don't know which work best in pregnant individuals, let alone which factors might predict response to treatment. But in the next few years this should change. I am lucky to have been working in the team running the Giant PANDA trial during my PhD. Here is a quick summary of why this trial is going to have clinical impact: - By randomising 2600 women to nifedipine v labetalol for treatment of high blood pressure in pregnancy, the trial will answer the question of which drug works best for mum and baby. - Designed to be inclusive. The original design and projects within the trial are focused on ensuring the trial population is representative of who is affected by high blood pressure in pregnancy, not limited to but including women with multiple long-term conditions and women of Black and Asian backgrounds. This is so important to make sure the results are relevant across women and people AFAB - not just specific groups. - We are running an embedded mechanistic study (cutely named BABY PANDA ), which will allow us to investigate drivers of response to labetalol and nifedipine in more detail. Using urine and blood biomarkers, we hope we can start to understand why these drugs work well in some individuals and not others. What has been really eye-opening for me as we have worked on this trial is how many clinicians (Obstetricians) across the UK feel strongly that nifedipine works better in women of Black ethnic backgrounds, to the extent they have been hesitant about offering participation in the trial. To be clear, we have no data in pregnancy to know ANY drug is better overall, let alone in a subgroup of people. But it is clear that there is a huge influence of general adult high blood pressure (hypertension) guidelines, as highlighted below by the brilliant Joel, on Obstetrics. My response has always been this: We don't have data to know which drug works best in pregnancy. This trial is going to provide definitive evidence. We need that evidence to work for women of ALL backgrounds, and especially populations disproportionately affected by high blood pressure in pregnancy. By being selective in who you offer the trial to, you potentially leave a whole swathe of women without data and so without answers. In the United States, the first line treatment for Black patients with high blood pressure is different than all other races, and this race-based approach may be missing the mark. Studies show that factors like multiple medications, medication adherence, and lifestyle changes matter more than race. When we rely only on race as a factor for prescription, we miss out on an opportunity for achieving better health outcomes for patients.
