Here is the breakdown of #Match2024 for #FamilyMedicine from the American Academy of Family Physicians: ✅ 4,595 medical students and graduates matched to family medicine residency programs. Here's the breakdown: 1,535 U.S. allopathic medical school (MD) seniors 1,493 osteopathic medical school (DO) seniors 749 U.S. international medical graduates (IMGs) 706 foreign IMGs 68 previous graduates of U.S. MD-granting schools 44 previous graduates of DO-granting schools ✅ Family medicine offered 5,231 positions, 124 more than in 2023, and 13.6% of positions offered in all specialties. ✅Family medicine residency positions were filled almost equally by U.S. MD seniors, DO seniors, and IMGs. Match results were fairly stable year-over-year for U.S. MD seniors, DO seniors, and U.S.-citizen IMGs. ✅The number of U.S. MD seniors matching increased by 36 compared with the year prior. It remains significantly below—805 matches—the historical peak of 2,340 in 1997. Only 8.3% of matched U.S. MD seniors matched in family medicine. ✅The number of DO seniors matching to family medicine decreased by 21. The percentage of DO students matching to family medicine (20.1%) declined from 22.2% the year prior, more sharply than the historical trend. ✅Almost one-quarter (23.5%) of U.S. IMGs and 12% of foreign IMGs who matched to any specialty matched to family medicine. The number of foreign IMG applicants who matched in family medicine rose significantly compared with 2023 (706 vs. 562). ✅A total of 3,140 U.S. MD and DO seniors and graduates matched in family medicine. ✅Family medicine represents 12.8% of all U.S. students or graduates who matched in 2024. ✅796 family medicine categorical programs offered 5,213 and filled 4,577 positions. The overall fill rate for family medicine-categorical programs was 87.8%.

At Dell Children's Medical Center, we are dedicated to rigorously reviewing scientific evidence and integrating it into our clinical practices. This integration takes the form of clinical standards, and meticulously crafted statements that outline the essential steps in caring for patients with specific clinical conditions. The purpose behind these standards is twofold: to minimize unwarranted variations in patient care, thereby enhancing patient outcomes, and to reduce healthcare costs. By leading the EBOC program and working in tandem with physicians and clinicians, the Evidence-Based Outcomes Center has successfully formulated 30+ clinical standards for a wide array of pediatric medical conditions. These clinical standards for pediatrics are crafted through a rigorous process involving a comprehensive review of evidence using the GRADE methodology, the incorporation of clinical expertise, and a thorough evaluation of potential benefits and risks. Our ultimate goal is to elevate the overall quality of healthcare by improving patient outcomes and concurrently curbing costs. In my role, I actively contribute to the development of evidence-based guidelines and evidence-informed pathways that offer comprehensive guidance spanning the entire continuum of care for specific diseases, ensuring that healthcare providers have a comprehensive roadmap to follow in delivering the best possible care to our young patients.

A STORY - Founder and CEO, medical Student Rotation LLC Dr. Munavvar Izhar founded Medical Student Rotation LLC (MSR) in 2022. Dr. Izhar is an Internist, Nephrologist, and Transplant Specialist. He was the Chief of Nephrology at various teaching hospitals in Chicago and now he is involved in Graduate Medical Education (GME) for practicing physicians, nephrologists, residents, and fellows. A few years ago, he and his colleagues (most of whom are chairmen or heads of departments) noticed that numerous entities in the US were charging exorbitant fees for outpatient and clinic rotations from medical students. Outpatient and clinic rotations are not considered very favorably by program directors. Even the hospital-based rotations with general attending physicians were charged $2500-$4000 per rotation by these entities. According to Dr. Izhar and his group, this was a blatant exploitation of students and hence he and his group floated Hospital-Based, Hands-On rotations with Faculty like the Chairman, Heads, and Chiefs of departments at very reasonable prices of $1500-$2000. Within a year of starting, MSR has been very successful in getting medical students matched into residency programs. This is only because the value of our rotations lies not only in them being hospital-based but also in being under the Chair, Chiefs, and Heads of the departments at teaching institutions. With the prices of our rotations being almost half of the others and our rotations being much better, the students are spreading our goodwill by word of mouth. What started a year ago as an offering of 3-4 rotations in Chicago has now increased to 50-60 rotation options extending into New York, Dallas, and Houston. Our commitment to keep the prices of rotation low and the standard of rotation high remains. Dr. Izhar believes in helping FMGs and IMGs achieve their dreams of getting into a residency program in the USA. His motto is that "In their success lies our success". The AAMC annual Report on Residents shows an increasingly diverse cohort of medical residents, trends within specialties, and preferred locations for physicians to practice after residency, among other findings. The report, which the Association of American Medical Colleges compiles annually, is intended to help residency applicants, program directors, residency specialty organizations, and researchers understand changes in trends. Among the findings: Changing demographics In the 2022-2023 academic year, there were nearly 155,000 residents, continuing the average increase of 5,000 residents each year since 2019. The percentage of women represented across all specialties slightly increased, from 47% in the 2021-2022 academic year to 48% in 2022-2023. Among residents and fellows with MD degrees, the percentage of those identifying as Hispanic, Latino, or of Spanish Origin increased slightly from 8.1% to 8.3%; and the percentage of those identifying as Black or African American also increased slightly, from 6.1% to 6.3%. Specialty trends The number of residents in the Internal Medicine subspecialty of Geriatric Medicine has decreased over the last four years, from 229 in 2019-2020 to 204 in 2022-2023. Two of the largest specialties, Emergency Medicine and Obstetrics and Gynecology, saw an increase in their numbers. Life post-residency A strong majority of the residents (57.1%) who completed their residency training between 2013 and 2022 went on to practice in the same state or territory as their residency. The states/territories that retained the highest percentage of residents were California (77.5%), Puerto Rico (74.8%), and Alaska (70.8%). The states/territories with the lowest retention rates were the District of Columbia (38.5%), Delaware (39.2%), and New Hampshire (40.8%). 𝗪𝗵𝗲𝗻 𝘁𝗵𝗶𝗻𝗴𝘀 𝗴𝗲𝘁 𝗰𝗵𝗮𝗹𝗹𝗲𝗻𝗴𝗶𝗻𝗴 𝗮𝗴𝗮𝗶𝗻 🥴    A medical center in Berlin, specializing in vascular medicine. The team is well-coordinated: studies are a daily routine here. Both interventional and observational studies are conducted at the national and international levels. The chief physician has been working with a permanently employed Study Nurse for several years. Conditions that seem perfect, as the Camovis team reflects on the deployment…    The medical center had played its trump card. The card with the broad yellow border and the radiant Joker in Camovis colors was designed by the company to capture the attention of sites that otherwise work with their own nurses. Almost like a personal backup before the success of a study starts to waver: when personnel resources become scarce, a Flying Study Nurse from Camovis steps in – precisely where the situation is tight at that moment.    Just like the situation in Berlin. The Camovis team was to support the study 18 hours a week to fill the emerging gap. The employed nurse was on parental leave, but her replacement had decided to pursue different professional paths after four months. A new replacement had not been found yet – and when the phone rang at Camovis in Charlottenburg, it was less than a week until the position would be vacant.    30 patients, a chaotic data situation, and a doctor with limited time awaited the Flying Study Nurse on Day One of the project. The interventional study focused on a medication designed to alleviate circulatory disorders in the extremities when used in addition to aspirin treatment. One patient group received the medication while the other received a placebo. Collaborative work was required, with precise documentation of how the subjects' health status behaved after administration.    After three days of orientation, the complete study back office was the responsibility of the Camovis employee. Additionally, there was the monumental task of data cleaning: the frequent turnover in the Study Nurse position had resulted in a far from consistent data situation. From 18 weekly hours, it escalated to 45 hours – over a period of nearly 6 months until the handover could ultimately be made to the medical center's new Study Nurse.    Satisfied faces rewarded the efforts of the Flying Study Nurse. On the patients' and the site's side. Not only had the Joker done exactly what it had promised: the study continued successfully. Financially, Camovis had also extended its generosity. Throughout my academic career I have noticed that EMS has established a research framework and process that is second to none. When it comes to clinical outcomes, scope of practice and the application of medicine, EMS is on the cutting edge. Yet, once the subjects/topics move from the back of the bus to the real world, that research juggernaut almost disappears. Despite EMS retention levels never being high, there is very little academic research to figure out the issue. Kurth et al. (2023) said it best, “No comprehensive national dataset captures EMS leaving using consistent and replicable estimates that can be compared over time.” For an industry that is close to celebrating 50 years of service, the fact that no comprehensive study in EMS retention has been done should raise a few eyebrows. With 24 years within this profession, I have witnessed many fantastic advances within the scope of our practice. This drive to serve the public better, more efficiently and provide the highest level of patient care is what drove our industry’s research and development machine. Yet, when it comes to how our industry is led, we have progressed very little in almost half a century. Why has our industry not delved wholly into this ongoing issue? Now, I must clarify, I said led, not managed. We have many articles to help the EMS chief better manage their people, yet there has not been an academic paper on leadership and EMS for many years. Therefore, I must ask why? What are we afraid of hearing and/or seeing? To give full discloser, I am working on my Doctorate in Leadership with EMS being my sole focus. During this time, this enormous gap in research has become even more evident. I have had to go to Nursing. Physicians, Pharmacists and Physicians Assistant school and literature to find subjects that relate to EMS and Leadership. We have an amazing body of work in regard to our patient care, response methodologies, unit readiness and…finally, PTSD and the job. Yet, this robust research mechanism seems to stop once the bus is parked back at base. Today is National Physician Assistant Day: Physician Assistants' scope of practice in Canada encompasses a wide range of medical and clinical responsibilities, allowing them to provide comprehensive and patient-centered care under the supervision of licensed physicians. As a 2nd year student at the University of Manitoba, Lauren is gaining practical experience through clinical rotations in medical specialties including Family Medicine, Emergency Medicine, Internal Medicine, Psychiatry, Geriatrics, Women’s Health, Paediatrics, Surgery and more. These rotations take place in hospitals, clinics, and other healthcare settings, where Lauren works alongside healthcare professionals, applying her knowledge in real-world scenarios. ⭐️ One of the key strengths of Physician Assistants lies in their ability to work across a wide range of medical specialties, making them valuable assets in various healthcare settings. ➡️ Improved Patient Satisfaction - PAs work in collaboration with supervising physicians, and this fosters comprehensive and personalized patient care. ➡️ Reduced Wait Times - With a PA on the team, the practice or department can accommodate a higher volume of patients, reducing wait times for appointments and medical services. ➡️ Enhanced Efficiency and Workflow - PAs are trained to perform clinical tasks, such as taking medical histories, conducting physical exams, interpreting diagnostic tests, formulating a diagnosis and differential diagnosis, formulating treatment plans, performing interventional or diagnostic procedures, prescribing medications, and engaging patient education. Attention all Global Family Medicine educators, champions, friends and advocates! Call For Papers— Global Family Medicine Education Theme Issue Call for Papers Submission deadline: November 15 Family Medicine seeks submissions for its upcoming theme issue on models of family medicine education around the world. Regardless of location, global family medicine educational programs face many similar challenges. Whether based in settings where family medicine is a novel specialty, or in areas where family medicine is long-established, faculty and academic units often struggle to advocate for appropriate resources and political support to remain viable and community responsive. Additionally, there are many challenges (ex: climate change and planetary health, burnout, and retention) for which we face a common need to innovate in order to ensure adequate family medicine education regardless of our country or region. For this theme issue, Family Medicine welcomes submissions that focus on successful efforts to advocate for family medicine education and training within local and global health systems; impact assessment of family medicine on populations served; barriers to family medicine training and evidence-based solutions to navigating those barriers; and creative partnership approaches to strengthen training between education programs located across international borders. Original research and brief reports are welcome. Narrative essay submissions should share original stories from the perspective of students, residents, faculty members, or practicing physicians. FM Focus submissions must have high yield, concise information that will be published as an infographic. Word limits and structure for the above manuscripts should follow those detailed in our author instructions. As a medical professional who has worn many hats throughout my career, I can confidently say that being a Family Medicine Doctor or General Practitioner is much more than just being the first point of contact for a sick patient of any age. While the bookish definition may be accurate, in the ideal world, a Family Medicine Doctor should possess a wide range of skills beyond just medical knowledge. In my experience as a Fellow in Family Medicine, I have found that the best General Practitioners are excellent communicators who possess an abundance of empathy. They are able to relate to patients and their problems, whether they are medical, psychological, or familial. They serve as a patient's advocate and advisor, acting as a wise guide and mentor throughout their healthcare journey. Furthermore, a Family Medicine Doctor should be a skilled problem solver, with an abundance of patience and the time to truly listen to their patients. They should possess the curiosity of a detective and be a thorough investigator, eliciting all necessary information to make informed decisions about their patients' health. Perhaps most importantly, a Family Medicine Doctor should be a specialist in all aspects of medicine. This includes Neonatal, Paediatrics, Surgery, Medicine, Emergency Medicine, Gynae, Geriatrics, and End of Life Care. No other sub-specialties or super-specialties offer the same range of qualities as a Family Medicine Consultant or Doctor. In conclusion, being a Family Medicine Doctor or General Practitioner is much more than just a title. It is a calling that requires a unique set of skills, including excellent communication, empathy, problem-solving, and a deep understanding of all aspects of medicine.

Intelligent planning and action are urgently needed to increase number of family physicians in provinces across Canada. "Ontario, Quebec & Alberta lead record family medicine residency vacancies". 268 family medicine residency positions remained unfilled across Canada after the first round of the 2023 match earlier this year. This is the highest number ever. Family medicine accounted for 76% of all vacant residency spots. Ontario had the highest number of family medicine vacancies with 100 unfilled spots, followed by Quebec with 99 vacancies & Alberta with 42. BC had the fewest with only 2 empty spots. Issues we must address to resolve this growing crisis: 1. Radical overhaul of primary care system is urgent & critical 2. Correct med school culture where messages may be given that family medicine is a less desirable choice 3. Increase transition to team-based primary care models, which provide better work-life balance for doctors, enable better delegation & deliver better health outcomes 4. Fee for service model is not working - fix it 5. Look at where things are working better and learn / adopt wining strategies. Example: B.C. introduced new longitudinal payment model earlier this year that works better for both patients and doctors - aligning benefits 6. Better consultation with family physicians by provincial governments - hard lesson learned from Alberta fiasco 7. Reduce the paperwork / administration burden on family physicians. Get with the times in using technology more effectively 8. Demonstrate that we genuinely value the role of family physicians and that we recognize the increased complexity they face.
