Rebecca Davis was enjoying a cruise with her husband last October when she realized something was very wrong. While walking to a port to do some sightseeing, she needed to pause and rest her legs several times. Back home, she started noticing mundane tasks become more challenging. “I got to where I would go down my basement steps to the laundry room, back up the steps and then had to sit down and take a break because my legs were tired,” she said. For someone who doesn’t like to sit still, this fatigue was alarming. Rebecca had a pretty good idea of what might be wrong, because her mother had been treated for peripheral artery disease (PAD). Test results confirmed her suspicions: blockages were reducing blood flow to both her legs. A 45-year-old man presents to the clinic with a 6-month history of progressive dysphagia. Initially, he had difficulty swallowing solids, but now he struggles with both liquids and solids. He reports frequent episodes of regurgitation, especially at night, and notes a significant unintentional weight loss of 10 pounds over the past three months. He denies any chest pain, heartburn, or odynophagia. The patient has no significant past medical history and does not take any medications. He does not smoke, drink alcohol, or use illicit drugs. On physical examination, the patient appears thin but is in no acute distress. The cardiopulmonary examination is unremarkable, and the abdominal examination reveals no abnormalities. According to the Practice Standards for Electrocardiographic Monitoring in Hospital Settings, in general, the mechanisms of arrhythmias are the same in both adults and children. However, the ECG appearance of the arrhythmias may differ due to developmental issues such as heart size, baseline heart rate, sinus and AV function, and automatic innervation. Advanced Life Support: Will REBOA feature in the future? REBOA is a technique used to occlude the AORTA and was initially used to assist with major trauma and haemorrhage control in those with abdominal and lower limb injury. As the attached research piece discusses there may be positive benefits for non-traumatic cardiac arrest. The benefits cited are: 1: Stimulation of the Baroreceptor reflex 2: Increased Coronary perfusion pressure 3: Could counteract the negative effects of current adrenaline dosing. Whilst this is a new and novel approach and more evidence is required, I feel it is important as clinicians to "scan the horizon" and see how practice might change in our own settings. I think having read the piece that it may show some benefit for certain patients who can be attended by highly skilled practitioners quickly, there will always be patients where this may not be appropriate. Resuscitative thoracotomy as an approach to penetrating cardiac trauma: A case report Introduction Cardiac trauma is a significant cause of death. Thoracotomy is an advanced resuscitation technique aimed at restoring cardiac output by relieving cardiac tamponade and repairing cardiac injuries. Objective A case report of a patient who suffered a penetrating precordial wound that was resolved with a resuscitative thoracotomy was conducted. A narrative review of the current state of the art is presented. Conclusions Thoracic trauma associated with cardiac injury is a potentially lethal condition. Despite early diagnosis, overall survival is anecdotal. Medical teams familiar with advanced resuscitation techniques such as resuscitative thoracotomy are required.

7 years old female Golden retriever, referred from another clinic for cardiac check up. Looking at previous chest radiography show multiples, prefipheral rounded nodular interstitial pattern which indicates pulmonary metastases. the dog came lethargic, inactive, and refuse to stand up, tachypnea, physical exam shows, reduced CRT, weak pulse, tachycardia 140bpm and reduced cardiac shock during auscultaion other are unremarkable. all of that me curious as you while reading so I scanned the spleen before Echocardiography. splenic mass heterogeneous cavitated mass in the tail of the spleen measures approximately 5.6*5.7cm. Echocardiography show echogenic mass in the pericardial sac with moderate pericardial effusion causing cardiac tamponade. pericardiocenesis: drained 150 ml of sanguinous fluid, followed by IV fluid bolus and the dog became more Bright, Actice and Responsive. Putting ainformations together makes diagnosis of malignant hemangiosarcoma on the top of DDX. #abdominal_Echography #small_animal_Echocardiography #small_animal_Cardiology
