Tectonic and therapeutic keratoplasties part 2: fungal keratitis can be very challenging to treat. Those aggressive infections which don’t respond to intensive anti-fungal treatment and continue to spread, threatening to reach the sclera, need a therapeutic keratoplasty, frequently a large size one, including some scleral margin. This is only the first step to control the infection; postoperative medical management to avoid recurrences is key and often challenging. Dr Giles Halliwell said: ‘’Urinary catheters are used in many common operations to allow the bladder to drain and are life savers, but they do cause UTIs in about a eighth of patients in the US and UK - about a quarter in developing countries. The timing of these interventions is key, with timely action especially important in cases like severe abdominal and necrotizing skin and soft infections. Procalcitonin to guide duration of therapy 🤓 Never utilized Procalcitonin to assist in determining duration of therapy. Most simple infections can be treated with five to seven of days of antibiotic therapy based on the current guidelines. Possibly find utility in more complex cases? Not sure how much less I’d be willing to recommmed for simple infection, but interested to watch and learn from further research and insight.

This is a case of Burkholderia cepacia right ear infection and invasive cholesteatoma. The real challenge is the fact that Burkholderia cepacia is multirresistant only responsive with cefepime or ceftazidime. Interestingly only one or two cases of Burkholderia cepacia ear infection have been published in the literature.
