Tectonic and therapeutic keratoplasties part 2: fungal keratitis can be very challenging to treat. Those aggressive infections which don’t respond to intensive anti-fungal treatment and continue to spread, threatening to reach the sclera, need a therapeutic keratoplasty, frequently a large size one, including some scleral margin. This is only the first step to control the infection; postoperative medical management to avoid recurrences is key and often challenging. I’ll be sharing tips for this and other tectonic keratoplasties during our course at the ESCRS 2024 meeting. #cornea #cornealgraft #cornealtransplant OCL Vision.

Procalcitonin to guide duration of therapy 🤓 Never utilized Procalcitonin to assist in determining duration of therapy. Most simple infections can be treated with five to seven of days of antibiotic therapy based on the current guidelines. Possibly find utility in more complex cases? Not sure how much less I’d be willing to recommmed for simple infection, but interested to watch and learn from further research and insight.
Procalcitonin's role in ruling in or ruling out bacterial infection is overrated. However, its potential for guiding the duration of therapy may be under appreciated and underutilized. 🎩 tip to the authors. https://lnkd.in/d9dq5fWW

This is a case of Burkholderia cepacia right ear infection and invasive cholesteatoma. The real challenge is the fact that Burkholderia cepacia is multirresistant only responsive with cefepime or ceftazidime. Interestingly only one or two cases of Burkholderia cepacia ear infection have been published in the literature.
