𝗠𝗶𝗰𝗵𝗶𝗴𝗮𝗻'𝘀 𝗜𝗻𝗻𝗼𝘃𝗮𝘁𝗶𝘃𝗲 𝗔𝗽𝗽𝗿𝗼𝗮𝗰𝗵: 𝗧𝗵𝗲 '𝗜𝗻𝗯𝗼𝘅𝗼𝗹𝗼𝗴𝗶𝘀𝘁' 𝗣𝗶𝗹𝗼𝘁 In a recent meeting, we confronted a common issue: physicians often struggle to finalize office visit documentation, close encounters, or pass responsibilities to residents. This creates billing challenges, as incomplete documentation hinders our ability to charge for services. A similar conversation unfolded on LinkedIn, highlighting the physicians' perspective. Many are overwhelmed by paperwork and time constraints, disrupting their work-life balance and leading to penalties despite their best efforts. Consider the case of a neurosurgeon who generates more revenue in the operating room than in the clinic. Naturally, they prioritize surgical procedures, leaving residents to manage follow-up visits. However, residents can't bill for these visits without an attending physician present, resulting in a continuous loss of revenue. Hiring an APRN or NP for these visits could be a viable solution. In response to these challenges, a Michigan hospital introduced the role of 'inboxologists' to alleviate the administrative burden on physicians. This innovative system triages incoming messages, ensuring that only essential communications require a physician's attention. Queries like appointment timings, test orders, or dietary restrictions before tests can be handled by support staff. This approach has shown promise, effectively streamlining communication and allowing physicians to focus on critical tasks. It's a refreshing strategy that could potentially revolutionize the balance between patient care and administrative duties. I am eager to see more healthcare systems adopt similar initiatives. In reflecting on recent feedback from "younger" patients at my practice, I've recognized a dual challenge: adapting our communication and operations to meet modern expectations, while also fostering a sense of accountability and understanding among patients. Two instances stood out: 1️⃣ Communication Gaps: A patient's distress over not receiving an imaging report reveals the gap between traditional healthcare communication methods and the instant, digital expectations of today's younger patients. While we strive to modernize communication, it's also important for patients to understand the complexities and limitations of medical processes. This patient had a MRI at an outside facility, we never received a report from the hospital, but the patient expected us to follow up on an MRI that they independently scheduled on their own. 2️⃣ Managing Expectations: Another young patient's dissatisfaction with a slight delay (10 minutes) and their reaction to being corrected on a self-diagnosed condition highlights the need for mutual respect and understanding. While we aim to respect and validate patient research, it's crucial for patients to also respect the expertise and constraints healthcare providers face. I welcome thoughts on how we can collectively work towards a more informed and respectful healthcare environment, especially when treating different generations, whether a gen-Z or baby boomer.
