Today on BackTable MSK: Cervical and thoracic epidural steroid injections (ESIs) can offer relief for patients with chronic pain; however, these procedures can carry serious risks. Providers should be well-trained to anticipate and mitigate these risks before treating patients with ESIs. In this Part II of our discussion on ESI's, Dr. Christopher Beck and I dive into various techniques and considerations for performing cervical and thoracic ESIs. We discuss the importance of trajectory and level aiming, the differences between interlaminar and transforaminal approaches, and the nuances of cervical and thoracic ESI procedures. The episode also covers patient positioning and potential complications. HCA Florida JFK Hospital is pioneering the future of atrial fibrillation (AFib) treatment with a groundbreaking clinical trial utilizing pulsed field ablation technology. This innovative procedure has the potential to revolutionize how electrophysiologists approach the treatment of AFib, ultimately enhancing patient outcomes and quality of life. AFib is the most common type of heart arrhythmia, affecting close to three million Americans, with that number projected to reach over 12 million in 2030. Treating AFib can significantly improve a patient’s quality of life, lowering their risk of stroke, heart failure, and other cardiovascular complications. Dr. Robert Fishel, Medical Director of Cardiac Electrophysiology at HCA Florida JFK Hospital, performed the procedure and is the lead investigator in the clinical trial. Acknowledged as one of America’s top 50 hospitals for cardiac surgery by Healthgrades, JFK Hospital stands at the forefront of cardiovascular excellence in the region. With the conclusion of enrollment in JC Medical’s Early Feasibility Study (EFS) evaluating the J-Valve for symptomatic patients at high risk for cardiac surgery with severe aortic regurgitation, I wanted to share a few reflections: 1. While research in the US can present challenges in the post-COVID environment, the dedication and efforts of the research teams exceeded all expectations. From the study’s National Principal Investigators (PIs) and site PIs to the research nurses to the imagers and hybrid teams, the expertise and commitment to help advance new technologies for patient care were truly commendable. 2. The commitment of JC Medical’s field team, clinical team members, quality, manufacturing, R&D, operations that made the trial successful in such a short period of time is greatly appreciated and inspiring. Talent is always important, but there are intangibles that become critical in high-pressure situations that cannot be understated. 3. The FDA’s review branch for heart valve therapies is working hard to ensure public health and are truly committed as an organization to working to help advance new patient technology. 4. Cardiac Imaging is so critically important as we move to treat new pathologies like the transcatheter treatment for AR. As Cardiac MR becomes a more utilized tool and part of the valvular heart disease guidelines, we will be able to better understand to which patient populations we should direct our therapies. 5. Patient input into research efforts, as well as their trust, remains integral to the process. 6. The Cardiovascular field has always been an evidenced-based practice with great interest to move to new heights to improve outcomes. We can be proud that the commitment is very real and continues to be strong. #tavi #fda #AR#J-Valve And....The cake was great!

This study explores geographic differences in ultrafiltration (UF) use during cardiopulmonary bypass in the US, analyzing data from 92,859 cardiac surgeries across four regions. Results reveal significant regional disparities in UF usage and volumes, with the Northeast and West employing UF more frequently than the Midwest and South. #CardiacSurgery #Cardiopulmonarybypass #GeographicVariation #Ultrafiltration

This week's Case of the Week is in honor of May being Stroke Awareness Month. A 70-year old man with a past medical history of a recent stroke status post thrombectomy presented to the emergency department with a sudden onset of left upper and lower extremity paralysis as well as facial droop with right-sided gaze preference. Patient's wife had quickly recognized the symptoms of an acute stroke and had called 9-1-1. ED physician ordered a CT brain, CTA brain, CTA neck, and CT brain perfusion using the stroke alert protocol. NIH Stroke Scale/Score (NIHHS) score was 15. One of our diagnostic radiologists, Dr. James Banks, MD, diagnosed the patient with a right carotid terminus thrombosis (blood clot) and carotid bifurcation stenosis. There was an associated middle cerebral artery territory core infarct and large surrounding ischemic penumbra that needed to be treated quickly in order to reverse any potential brain damage. Patient was not a tPA candidate. Patient was immediately sent to the biplane room for a thrombectomy and possible internal carotid artery (ICA) and common carotid artery (CCA) stenting. Dr. Alejandro Santillan, one of our neurointerventional surgeons, performed an ICA and CCA reconstruction with two overlapping stents followed by thrombectomy of the right ICA terminus with complete reperfusion of the right hemisphere. Thrombolysis in cerebral infarction (TICI) of 3. After the procedure, the patient had no neurological deficits. NIHHS of 0. Our clinic will follow-up with the patient now that he has been discharged to ensure he receives regular brain scans to look for new or developing stenosis that could cause additional strokes.
