📃Scientific paper: Computer-assisted evaluation enhances the quantification of interstitial fibrosis in renal implantation biopsies, measures differences between frozen and paraffin sections, and predicts delayed graft function Abstract: Background (Pre-)Implantation biopsies provide important data on the quality of donor kidneys. Interstitial fibrosis, as a known predictor for kidney disease progression, is an essential feature of this evaluation. However, the assessment of frozen sections of implantation biopsies is challenging and can result in the disposal of candidate organs. We sought to apply digital image analysis (DIA) to quantify the differences between frozen and paraffin sections when evaluating interstitial fibrosis, identify factors that influence these variations and test the predictive value of the computerised measures. Methods We quantified the differences between frozen and paraffin sections in the same biopsy samples by measuring Sirius red-stained interstitial areas (SRIA) in DIA. We compared them to the original reports, and retrospectively correlated our findings to clinical data, graft function and outcome in 73 patients. Results Frozen sections display a broader interstitial area than paraffin sections, in some cases up to one-third more (mean difference + 7.8%, range − 7 to 29%). No donor-related factors (age or gender, cold ischemia time, or non-heart-beating donor) influenced significantly this difference. Compared to the original assessment of frozen vs paraffin sections in optical microscopy, the DIA of interstitial fibrosis shows a higher consistency (ICC 0.69). Methods PubMed, Ovid Medline, Ovid EMBASE, Scopus, and Web of Science were screened for a comprehensive literature review from 1990 to 2020 regarding series of patients treated for CS-dAVF with endovascular approaches. We performed a proportion meta-analysis estimating the pooled rates of each outcome also including data of patients treated in our center. Results A total of 22 studies reporting 1043 patients and 1066 procedures were included. Chemosis was reported in 559 out of 1043 patients (45.9%), proptosis in 498 (41.5%), and ophthalmoplegia in 344 (23.5%). A transvenous embolization was preferred in 753 cases (63.2%) and coils were used in 712 out of 1066 procedures (57.8%). Overall, 85% (95% confidence interval, CI 69.5–96.1%) of patients had a complete resolution of symptoms, while complications occurred in 7.75% (95% CI 3.82–12.7%) with minimal permanent deficits (0.15%). 🔬 𝗡𝗲𝘄 𝗣𝘂𝗯𝗹𝗶𝗰𝗮𝘁𝗶𝗼𝗻 - '𝗦𝘂𝗯𝘁𝗵𝗿𝗲𝘀𝗵𝗼𝗹𝗱 𝗿𝗲𝗷𝗲𝗰𝘁𝗶𝗼𝗻 𝗮𝗰𝘁𝗶𝘃𝗶𝘁𝘆 𝗶𝗻 𝗺𝗮𝗻𝘆 𝗸𝗶𝗱𝗻𝗲𝘆 𝘁𝗿𝗮𝗻𝘀𝗽𝗹𝗮𝗻𝘁𝘀 𝗰𝘂𝗿𝗿𝗲𝗻𝘁𝗹𝘆 𝗰𝗹𝗮𝘀𝘀𝗶𝗳𝗶𝗲𝗱 𝗮𝘀 𝗵𝗮𝘃𝗶𝗻𝗴 𝗻𝗼 𝗿𝗲𝗷𝗲𝗰𝘁𝗶𝗼𝗻' 🌟 Our recent study analyzed genome-wide microarray data from 5,086 kidney transplant biopsies. It revealed that many biopsies currently classified as “no rejection” may actually harbor subthreshold levels of T cell-mediated rejection (TCMR) and antibody-mediated rejection (ABMR). This subthreshold activity had implications for future rejection episodes and was associated with increased risk for graft loss. Key Findings: • 𝗦𝘂𝗯𝘁𝗵𝗿𝗲𝘀𝗵𝗼𝗹𝗱 𝗔𝗰𝘁𝗶𝘃𝗶𝘁𝘆: There is an increasing gradient of molecular classifier scores present in biopsies deemed free of rejection (by histology, by molecules, or both). We found a gradient of increasing subthreshold rejection activity (ABMR and TCMR) in biopsies called no rejection, TCMR activity in biopsies called ABMR, and ABMR activity in biopsies called TCMR.  • 𝗖𝗹𝗶𝗻𝗶𝗰𝗮𝗹 𝗜𝗺𝗽𝗹𝗶𝗰𝗮𝘁𝗶𝗼𝗻𝘀: Biopsies with elevated subthreshold rejection activity were linked to increased risk for graft loss and reduced renal function. Furthermore, these findings predict a higher likelihood of actual ABMR and TCMR in future biopsies. • 𝗜𝗺𝗽𝗮𝗰𝘁 𝗼𝗻 𝗣𝗮𝘁𝗶𝗲𝗻𝘁 𝗖𝗮𝗿𝗲: This study suggests that increased monitoring strategies may be needed for cases called 'no rejection' but with elevated subthreshold rejection activity, as this state may be less benign than previously thought. Pleased to announce that our study entitled 'Regenerative treatments for scleroderma in cutaneous manifestations of the face: a systematic review’ is now published in Clinical and Experimental Rheumatology! Scleroderma is a heterogeneous chronic autoimmune disease affecting connective tissue, characterized by chronic inflammation and fibrosis, particularly affecting internal organs and skin. Orofacial involvement is common, leading to facial atrophy, mask-like appearance and difficulties in function that significantly impact patients’ quality of life. Our study evaluates different autologous regenerative treatments of facial manifestations of scleroderma, aiming to provide comprehensive understanding of their effectiveness in reducing fibrosis, and thereby improving function and skin quality. Treatments consisted of autologous fat grafting, platelet-rich plasma, stromal vascular fraction, and adipose-derived stem cells. Our results show that most studies showed improvements of symptoms, but no treatment was considered superior. Autologous regenerative treatments hold potential for alleviating cutaneous manifestations of the face in scleroderma. The start of a new month makes us once again turn the attention to the UK to review the published NICE guidance decisions for May. The UK is the third largest European healthcare market based on the size of their population. NICE is a key stakeholder for manufacturers looking to gain access in the UK, with NICE requirements often differing from those of the regulatory agencies prior to them. In May, NICE published a total of 24 guidance. Six of the technology appraisals corresponded to updates from previously published guidance that were not included in the present report. Most of the guidance published correspond to Oncology and haematology treatments.  Focusing on the remaining 18 sets of guidance published by NICE in May: 📈 NICE published across 5 different therapies: Oncology, Haematology, Endocrinology, Infectious diseases, CNS/Neurology and Dermatology. 🌐 10 treatments were recommended, seven of them including commercial arrangements. Half of the commercial arrangements correspond to simple patient access scheme (simple discount), and the other half to managed access agreement between the company and NHS England. ⛔3 treatments had the review terminated due to decisions from the manufacturing company; one from each of the following disease areas: oncology, haematology and dermatology. In May, there were 6 technology appraisals publications that correspond to updates from previous decision published as early as 2007. Half of the appraisals were in multiple sclerosis, and had changes relating to the definition of rapidly evolving severe multiple sclerosis. The other half were products where biosimilars have been launched, so updates relating to the patient access scheme and procurement of biosimilars has been updated. Two treatments received conditional recommendation, meaning they will be reimbursed during an evidence data generation period.
