The sharing is actually about discussing the issue of Human Error in Hospitals. Regarding this, a student conducted a case study on human error in the aspects of analytical laboratory work in hospitals. Analytical laboratory work involves diagnostic activities such as pathology, haematology and microbiology tests. The scope of hospital laboratory services covers specimen handling, blood, urine, throat swab, rectum, vagina and urethra specimen testing, sputum and stool (feces) specimens. The issue of human error in hospital laboratory activities can arise due to various human factors. As a simple framework it can be stated as follows; divided into three phases - human error during the 𝐩𝐫𝐞-𝐚𝐧𝐚𝐥𝐲𝐭𝐢𝐜𝐚𝐥 phase, 𝐚𝐧𝐚𝐥𝐲𝐭𝐢𝐜𝐚𝐥 𝐩𝐫𝐨𝐜𝐞𝐬𝐬 and 𝐩𝐨𝐬𝐭-𝐚𝐧𝐚𝐥𝐲𝐭𝐢𝐜𝐚𝐥. Pre-analytical errors are usually caused by issues such as unclear communication (communication gaps) between healthcare professionals, improper collection or handling of samples, and inaccurate labelling. Meanwhile, during the analytical phase, errors occur due to equipment failure, or not being calibrated, incorrect test selection, inaccurate pipetting, specimen contamination, and intrinsic aspects such as staff fatigue and workload. Then for the post-analytical phase, errors can occur during transcription or reporting of results. It is directly related to the issue of misinterpreting results received. Therefore, specific human factor interventions need to be tailored to the identified error issues. Engineering and administrative approaches are able to resolve these problems. The effectiveness of interventions depends on the approach taken, so gathering input from relevant laboratory staff is key. They are individuals who interact directly with the laboratory system and work processes. Their views can lead to appropriate, accepted and practical interventions. Clinical Trialist - Physicians: Exciting Opportunity for a Clinical Research Career at a Well-Established Center in the Indianapolis Area. Even though I have been a principal investigator for >35 years for 550 trials in the cardio-metabolic area (more planned this year), I am mystified that this career choice has been ignored by the medical community in general. This has been significantly hampered by being employed by one of the different hospital systems in town. As a private business: (Remember when physicians were independent and had their own practice!!) 1.    One does not have a non-compete contract. 2.    One gets to pick and choose the appropriate trials in which to participate. 3.    One is not forced to see 20-30 patients a day to keep the bean counters happy. 4.    One has access to the new medications that one hopes will have a positive impact for people’s lives. Here are a couple of examples which have been exciting for me: Regarding every new cholesterol lowering drug on the market, I have been in one or more of the trials prior to drug approval (230 lipid trials). Regarding the new obesity medications (the GLP-1 and GLP-1 combinations), I have been in 12 trials so far and will participate in several more new ones, still this year. So, if physician/independent medical group might want to get involved, I would love to discuss opportunities that would be available.
