I have just had a follow-up chat with Emerald Lake Safety about the video I have done regarding their free - to patients in the US only at this stage - test to determine if you are carrying one of the four genes that markedly increase your susceptibility to developing the JC virus. I have to say that I share their bafflement as to why pharma and doctors aren't biting their hand off for this. PML - Progressive Multifocal Leukoencephalopathy - is caused by the JC virus. Although the overall risk is small, in MS there is an index and generally speaking when a patient exceeds 2 the monitoring becomes a lot more rigorous or more often a switch of DMT is made. Regarding risk: although this is a rare thing the incidence is skewed toward patients with an autoimmune condition that bare being treated with monoclonal antibodies (MABs) with Natalizumab being notably higher risk. For a more day to day context you are far LESS likely to win a lottery jackpot than get PML if you are JC +ve and on natalizumab (Tysabri). Canine TB is on the rise, or at least in the UK according to this recent paper. A few takeaways: ➡️ M.bovis appears to overtake M. tuberculosis as causative agent ➡️ 44.1% of the cases were primarily pulmonary, 14.5% were primarily abdominal, and the remainder were disseminated or miscellaneous ➡️ diagnosis can be based on culture, ZN-stain on cytology, PCR or IGRA ➡️ over 90% of dogs treated with prolonged triple antibiotics had a good clinical outcome Consider it for granulomatous lesions! https://lnkd.in/eDKhrFB2
