It is important that the information you gather is then implemented to avoid reoccurrence and to continually improve your service. For example: Have you reviewed your processes as a result and made amendments? Have you changed your communication method? Have the risk assessments or care plans been changed? Have you provided the additional training that may be needed? Have you identified and analysed trends?
