The final episode (#8) of the COA in PROgress podcast is out! This one is a bit more forward looking, offering the fabulous John Fox and Olivier Wong an opportunity to reflect on how the HTA and healthcare systems are ready to embrace DHT, CAT and other novel ways of understanding treatment effects. https://lnkd.in/egA2wwMt; https://lnkd.in/esTPNdW6 Now that all 8 episodes have launched, I wanted to briefly reflect on my 5 key learnings: (1) The regulatory framework for COA offers clear opportunities for broad use of COA trial data in understanding patients, diseases, and treatments to inform regulatory decision-making. Payers are still catching up, in terms of defining evidentiary standards and structures for COA-derived insights/value; but COAs collected as part of routine care are now starting to be incorporated into clinical decision-making in some therapeutic areas. (2) For COA data to be considered as robust, insightful and interpretable, they must be informed by a carefully considered and detailed strategy including a solid hypothesis, an empirically informed measurement approach, and careful endpoint specification. (3) A strategy must include consideration around data collection (i.e. implementation of COAs) - selecting the right COA instrument to measure the relevant concept(s) is important; but it can easily be undermined if engagement with the instrument is poor and missing data is significant. This renders the data hard to interpret. (4) There is plenty more that can and should be done to make use of COA to help inform intervention development and the management of disease. Some of this pertains to further developing the measurement (e.g. briefer, adaptive instruments, global scales), some of this pertains to implementation (e.g. reactive COAs, passive monitoring), and some of it pertains to data interpretation, use and sharing (e.g. real-time analysis of intensive longitudinal data, dashboarding to inform clinical consultations). (5) It is harder to say "Boehringer Ingelheim" than you may think! I recorded 5 versions for the editors to choose from. But even with the one they chose many of you have taken delight at telling me how funny I sound trying to say it at the start of each episode!
