Every day, 750 older people are hospitalized due to serious medication-related side effects. My loved one had a 13th #medication added to their regimen today in their recovery after a major illness. With multiple hospitalists, 4 specialists, 1 PCP, & 1 NP all writing scrips, it's a struggle to maintain accurate records from one visit to another. We find ourselves being the ones keeping med records accurate. It shouldn't have to be this way. #collaborativecare agreements between doctors & #pharmacists can help address issues associated with #polypharmacy thru #MTM (#medication therapy management) insuring greater records accuracy, reducing potential #druginteractions and reducing #medicationcosts for patients, insurers & employers. #ipcrx #communitypharmacy
