Point-of-care testing (POCT) offers a revolutionary approach to kidney health assessment, particularly in areas with limited access to laboratory services. Implementing uniform preanalytical, analytical, and post-analytical quality criteria ensures consistent and reliable results across POCT devices for measuring creatinine and urine albumin. POCT devices for creatinine testing should generate glomerular filtration rate (GFR) estimates, aiding in comprehensive kidney health evaluation. POCT devices for urine albumin testing should assess the ability to accurately produce albumin-to-creatinine ratio (ACR) results, particularly in detecting significant albuminuria (ACR ≥30 mg/g or ≥3 mg/mmol). While POCT offers convenience and rapid results, its accuracy must be rigorously evaluated. It is essential to balance the benefits, including early diagnosis and cost-effectiveness, against potential harms, such as misdiagnosis. POCT's ability to use small sample volumes, like fingerprick samples, makes it particularly valuable for testing in children, ensuring timely and less invasive assessments. POCT holds immense promise in advancing kidney health, especially in underserved populations and remote settings. By ensuring accuracy, reliability, and accessibility, we're paving the way for earlier diagnosis, monitoring, and treatment of kidney diseases. The more Epinephrine in cardiac arrest, the better right? Ok, we know many of you don't believe that. By now many EMS agencies have limited Epinephrine dosing in cardiac arrest to 3 mg. Some have even limited it to 1 mg. A recent study looked at total Epi dosing, specifically in patients receiving eCPR ! 🚑💉 📊 Key Takeaways from the Study: ➡ Researchers classified patients into low (≤3 mg) and high (>3 mg) epinephrine dosage groups. ➡ Patients with a lower epinephrine dose had better neurological outcomes at hospital discharge (55%) compared to the high-dose group (24%). ➡ Higher doses correlated with unfavorable neurological outcomes, with an adjusted odds ratio of 4.6 for poor results, underscoring potential risks of high cumulative doses. 🧠 Conclusions: ➡ Higher cumulative doses of epinephrine (>3 mg) might be linked to worse neurological outcomes post-ECPR. ➡ This data suggests reevaluating epinephrine dosing during resuscitation, especially considering the potential for adverse neurological impacts.
