When a person is experiencing a behavioral health crisis, counseling is often just the first step in helping them find stability. Often, they need access to medical care and social services and someone they can trust to guide them through the process and act as their advocate. Individuals discharged from emergency rooms, psychiatric inpatient care, or the criminal justice system, as well as the unhoused, are especially vulnerable. The peer guidance provided by our Targeted Care Management Program gives them hope and a single point of support they can trust. If counseling were the only thing required to move people from stability to crisis, our work at BCCS would be much simpler. But people released from emergency rooms or inpatient psychiatric care, or from the criminal justice system, as well as those who have fallen through the social safety net, need a continuum of services. Equally important, they need someone who’s been through some of the same challenges and can understand how they’re feeling to guide and advocate for them. In our Targeted Care Management (TCM) program, that someone is a peer counselor. The personal, individualized approach of TCM is often the turning point in the lives of clients on the verge of losing hope.
