During the month of February, we will be sharing information about American Heart Month. Hospice of New Jersey provides a specialized level of care to hospice patients with heart diseases. Did You Know? Four out of five people don’t know they have heart failure until their symptoms are severe enough to put them in the emergency room. Heart failure is a condition in which the heart is not pumping blood throughout the body as well as it should. While heart failure may shorten a patient’s life, good medical management can help a patient live longer with a better quality of life. Early diagnosis gives doctors more time to find a treatment that can keep your symptoms in check and prevent other health issues. The symptoms of heart failure can come on very slowly, and they are easy to mistake for normal signs of aging. To support a hospice plan of care, the combined effects of the primary cardiopulmonary condition and any identified secondary condition(s) and/or co-morbidities should be such that most beneficiaries with the identified impairments would have a prognosis of 6 months or less. #AmericanHeartMonth #OurHearts #OurCareFamily
