We are excited to unveil a second set of new data presented at this year’s American Heart Association Scientific Sessions, showing that Cadence’s remote patient monitoring solution yielded meaningful improvements in vitals and adherence to life-saving medications and guidelines amongst patients with heart failure with preserved ejection fraction (HFpEF). In a study of 215 patients with HFpEF, results showed a 12x increase in patients on all four pillars of guideline-directed medical therapy (GDMT), a treatment that saves and improves the quality of lives for patients with heart failure. Dr. Marat Fudim, an advanced heart failure specialist and cardiologist at the Duke University Medical Center and senior author on Cadence’s AHA abstract, shared: “This data demonstrates how RPM can help clinicians titrate and achieve high doses of life-saving GDMT in an effective and timely manner. Cadence’s team of multi-disciplinary heart failure clinicians are leveraging RPM and GDMT protocols to support HFpEF patients achieve better cardiovascular outcomes and quality of life.” Read the full results and abstract here: https://lnkd.in/e8Jcqs7k #aha23

In today’s #MissionMonday, we’re highlighting the powerful impact Cadence has had on patients like Tori and her husband in managing their health together. Their journey underscores the importance of accountability and support in maintaining peace of mind. Introduced to Cadence through their doctor as part of our partnership with Montage Health, Tori, who previously relied on a manual blood pressure cuff, found it to be a game changer. “Using Cadence makes it so much easier to monitor my blood pressure daily, and knowing that someone else is keeping an eye on my health brings me comfort,” she shared. Tori’s husband has also seen remarkable changes thanks to Cadence. “Before, when I told him his blood pressure was too high or too low, he would sometimes dismiss it. Now, he’s much more engaged and reflective about his health. If his numbers fluctuate, he actively considers what he might need to change,” Tori explained. We love to hear and see how Cadence empowers couples to hold each other accountable, delivering not just better health outcomes, but also the peace of mind that comes from knowing they’re in it together.

The Nashville Health Care Council's #Sessions2024 discussion about the evolving needs of America's aging population included themes about creating access to the services seniors need, a greater focus on preventative care, building trust, and creating care models that intervene faster to address medical needs in the outpatient setting, preserving acute care for when hospitals are needed most. Community Health Systems CEO Tim Hingtgen noted that most of the one million patient encounters happening across CHS health systems each month are for outpatient care. He said that developing stronger patient-provider relationships and making it easier for seniors to connect to the various services they need are keys to better care and fewer hospitalizations for aging Americans. It was also the perfect time to talk about the benefit of CHS' at-home, remote patient monitoring and chronic care management program powered by Cadence. With more than 14,000 patients enrolled, CHS and Cadence are helping older adults live healthier, providing peace of mind for patients and their families, and we are achieving better outcomes. This afternoon Cadence founder and CEO Chris Altchek joined Lifepoint Health®’s David Dill, Ascension Saint Thomas' Fahad Tahir and TeamHealth’s Leif Murphy on stage at Nashville Health Care Council #Sessions2024. The group delved deep into the transformative power of collaboration in driving results for patients, communities, and health systems. Chris shared, “When it comes to these kinds of partnerships, instead of angling for ownership or interest, we take the approach of ‘we’re all in this together’ because it’s really not a zero-sum game.” Technology will have an even more profound impact on how healthcare is delivered in the coming decade. It’s not just about making care more efficient, it’s about making it more proficient. The future is about forming fewer but deeper partnerships, working alongside the best health systems to personalize guidelines and raise the standard of care at scale.

In a recent conversation with our Care Team she shares how helpful Cadence has been on her health journey: “The people I’ve connected with over the last two years have been wonderful. In her recent fireside chat at the MedCity News INVEST Digital Health conference, Dr. Cunningham addressed the overwhelming influx of patient data, much of which burdens physicians with unnecessary details.
