Masthead forestay stem ball fitting failure. Failed during heavy weather (in the marina) after the yacht sailed halfway across the Med(!). It can be seen how deep the crack was prior to failure. I know some riggers do not replace these fittings as part of a complete yacht re-rig which is nonsensical. We know stainless fatigues, and stress raisers are located at sharp turns, so it's obvious that these should be replaced.
