"Interventional procedures are at the core of our work. We know how to perform them, how to manage complications, and how often they occur. Our patients, however, may not. Sometimes, they don’t even know who we are or what interventional radiology entails. For this reason, we need to dedicate more time to our patients—to establish a strong connection with them before, during, and after the procedure. In our institution, a crucial moment for the success of the treatment is the ultrasound planning visit, where we strive to build a genuine connection with our patients by explaining the pros and cons of percutaneous treatment. Even just before the procedure and anaesthesia, a calming approach is essential for ensuring a positive experience for the patient. Finally, it’s important that patients don’t feel abandoned or handed off to another physician after the procedure. Taking the time to explain the results of our treatments and making patients aware of our clear follow-up pathway dedicated to them is the best way to ensure they receive long-lasting benefits and retain a positive memory of our specialty." In a study to compare the cost of standard intravenous conscious sedation with that of sedation with adjunct self-hypnotic relaxation during interventional radiologic procedures, it was found the use of adjunct hypnosis with sedation reduced the cost. Data was reviewed from a prospective randomized study in which patients undergoing vascular and renal interventional procedures underwent either standard sedation or sedation with adjunct hypnosis. The data was then used to construct a decision analysis model to compare the cost of standard sedation with the cost of sedation with adjunct hypnosis. The finding was interventional radiology saved an average of $338 per case due to shorter procedure times, fewer complications, and faster recovery. Presenting a successful Left Carotid Artery Stenting procedure performed using our First International Cathlab at the Comprehensive Interventional Center in Iraq. The procedure involved crossing the lesion with a 0.14 wire and employing over distal protection (6mm filter). Subsequently, the lesion was predilated with a 2mm NC balloon, followed by the deployment of a 6-8 mm * 40 mm stent, resulting in excellent procedural outcomes. Post-procedure, the patient experienced neurological improvement, highlighting the effectiveness of the intervention. “Transcranial Sonography Used as a Valuable Diagnostic Tool for Detecting a Hemoventricle, in an Intensive Care Unit Patient,” by Drs. In the current print issue of the Journal of Diagnostic Medical Ultrasound (May/June 2024) there is a nice case study report from two Argentinian doctors demonstrating an innovative use of POCUS TCD. You will learn about hemoventricles and why TCD (and ultrasound in general) is needed in the ICU. As the authors explain, “A hemoventricle is a rare and life-threatening condition, characterized by blood accumulating in the brain ventricles. Computed tomography (CT) is commonly used in the diagnosis of hemoventricle. However, the use of CT is limited in the intensive care unit. ... A case study is presented of a 64-year-old female patient with a ruptured aneurysm who underwent endovascular coiling and the placement of an external ventricular drain. Point-of-care ultrasonography revealed ventricular dilatation that was associated with hyperechoic content and that was consistent with hemoventricle.” Cheong and Tamagnone explain that a CT examination confirmed their diagnosis and conclude that, “This diagnostic process allowed for prompt surgical intervention, with the placement of an EVD. This case demonstrates that sonography of the brain can be considered a valuable diagnostic point-of-care tool and may be a first choice for diagnosing a hemoventricle that may have resulted from cerebral aneurysmal rupture.” Other controversies concerning possible treatment options and the limitations preventing an alternative use of MRI in the acute phase are discussed.
