Advancements in sports rehabilitation, particularly for ACL reconstruction (ACLR), have evolved significantly. The shift from a conservative approach, where leg extensions were delayed for up to three months, to a more proactive strategy with early initiation of exercises like leg extensions and straight leg raises (SLR) within the first few weeks post-op is notable. 1. Leg Extensions (Open Kinetic Chain): - Traditional Approach:** Waiting for three months before starting leg extensions was rooted in concerns about stressing the healing ACL graft. - Current Approach:Initiating leg extensions as early as the fourth week suggests a more progressive stance, focusing on controlled and guided movements to optimize recovery without compromising graft integrity. 2. Straight Leg Raises (SLR):Open Kinetic Chain Exercise. - Traditional Approach:Delayed initiation to reduce strain on the healing tissues, typically after several weeks. - Current Approach:Starting SLR within the first two weeks post-op indicates a shift towards early activation of the quadriceps and overall engagement of the lower extremity muscles. The move toward early incorporation of open kinetic chain exercises underscores a more individualized and evidence-based approach to ACLR rehabilitation. This shift is often supported by research indicating that controlled, early mobilization can contribute to improved outcomes without compromising the surgical repair. It's crucial to note that these advancements are a result of ongoing research, improved understanding of biomechanics, and a desire to optimize recovery timelines for athletes. However, individualized care remains paramount, and rehabilitation protocols may vary based on the specifics of the surgery, patient characteristics, and the surgeon's preferences.
