The Heart of Rome: after some weeks we come back with a new interesting case from the Diagnostic, Interventional and Acute Cardiac Care Unit of the Isola Tiberina Hospital Gemelli Isola. 85 yrs old very active man with NSVT and mild LV dysfunction. CAG showed intermediate pLAD disease and CTO of a probably important first diagonal branch with collateral flow from the same left system. We had a frank discussion of the case with the patient and we decided togheter to approach it invasively. After a failed attempt with a workhorse wire and with a Fielder XT, we were able to cross the CTO with a ASAHI INTECC Europe B.V. Gaia 1st. We used an ACIST Medical Systems Navvus II microcatheter to assess distal back pressure (cFFR 0.43). Then we assess pLAD and we found a significant stenosis (cFFR 0.77). Accordingly we decided to treat this bifurcation with a provisional approach using DES in main branch and DCB side branch. We recanalized the D1 checking the optimal post-POBA result using the physiological guidance and we applied a Medtronic Prevail 3.0x 30 DCB (final cFFR 0.94). Then, we predilated and implanted a Abbott Xience 3.5x 48 with a final POT (final cFFR 0.90). Follow up at 3 months OK (patient's consent to publication).
