📢 **Important Update for Home Health Providers!** 📢 The October 2024 quarterly refresh for the Home Health Quality Reporting Program is now live on Care Compare! 🎉 Key updates include: 🍄 **New OASIS and Claims-Based Measures:** They have updated the HH Outcome and Assessment Information Set (OASIS) and HH QRP claims-based measures based on the standard number of quarters. 🍄🟫 **Changes to Public Reporting:** We're saying goodbye to two claims-based measures: - Acute Care Hospitalization During the First 60 days of Home Health (ACH) - Emergency Department Use without Hospitalization During the First 60 Days of Home Health 🍄 **Star Ratings Update: ** The Home Health Quality of Patient Care Star Ratings now include the Home Health Within-Stay Potentially Preventable Hospitalization (PPH) measure, replacing the ACH measure. Guardiant Health works with managed care entities on post discharge medical care program with an assigned medical team that works with patients coming out of the hospital and the complex needs that follow. We have anecdotally noticed these trend in home health as well and fortunately have been able to mitigate this gap through our post discharge medical team.
