The potential of Institutional Special Needs Plans (I-SNPs) to enhance both clinical outcomes and financial sustainability in nursing homes is gaining well-deserved attention. As we continue the national shift towards value-based care, it's critical to explore innovative care models that prioritize patient outcomes while managing costs. As authors of a recent NEJM Group article note, “the financial risk taken on by these plans creates incentives for on-the-ground investments in clinical care.” This approach helps keep care in the nursing home setting, reducing unnecessary hospitalizations and improving care quality. Here at CareNu, we help providers embrace the opportunities and challenges of risk-based models, offering the tools, support and expertise necessary to navigate this evolving landscape. Risk isn’t something to be feared –– it's a catalyst for innovation and improvement in healthcare. And I-SNPs, with their focus on patient-centered, high-quality care, are a prime example of how taking on risk can lead to better outcomes for both residents and providers. #CareNuCares #CareNuIsNow #ISNP #ValueBasedCare #HealthcareInnovation

Why Did New York State Hospitals Rank So Poorly? Since 2015 we have noted the high pressure injury rates of NYS hospitals and we’ve noted the poor quality performances of our neighborhoods’ hospitals. How can this be when so much time and attention is placed on improving quality? Not surprisingly one particular problem was noted - “pressure ulcers” that one confounding problem that, in some hospitals, seems to defy efforts using preventative measures. We founded Care Improvement Strategies LLC to attempt to increase the focus of our area hospitals on the importance of accurate assessments and timely and consistent preventative interventions. Thank you, Dr. McLoughlin, for your timely and comprehensive review of NYS hospital’s challenges. Why Did New York State Hospitals Rank So Poorly? Colleen C. Why Internal Medicine? In this video, Joseph, an RN on H81, talks about the opportunity to work with a wide variety of patients and patient needs, the close collaboration of teams, and the "great coworkers". There's good conversation and great teamwork, on the unit and during team outings. That's why, after three years on H81, he does not see himself leaving any time soon. Curious about H81 and how we can help you get in touch with the reason you got into nursing? Let's talk. #ClevelandClinic
