Tomorrow night at 10pm in 1988 the unthinkable happened and we owe it to everyone on board and their families to learn from the past. If you can take 1 hour out of your schedule in the next few days to learn, reflect and commit to implement improvement this video is a great place to start. How do you really know what is going on in your workplace? Can you really know it is safe? Brian Appleton in this video for the @BBC explains some of the softer elements of what went wrong on that fateful day in July 1988 on the Piper Alpha rig. I have watched this video many times and every time I find it useful and insightful and I take something new from it. However it is the closing statement which always sticks with me: "The sum and quality of our individual contributions to the management of safety determines whether the colleagues we work with live or die" It is not only the contributions on the day but the contributions in the days, months and years prior to the incident. Incidents don't happen to strangers they happen to the people we know and work with, maybe even to us. Contributions are not only important in their quantity but also in their quality. Each of us makes contributions to the management of safety at our facility whether we realise it or not. Often a poor quality contribution takes just about the same time as a good quality contribution (refer to the OIM who did nightly rounds on the Piper to keep his finger on the pulse or safety). Reflect on your contributions to the management of safety from design to operation. - Are you giving good quality contributions? - Is there a system or process in place which is checking the quality of your contributions and driving you to improve them? If not why not? - How could you improve the quality of your contributions and those of the people around you? - Have you done enough today to ensure that the colleagues you work with live?
