I fully appreciate the benefits of specifically identifying sepsis and being able to discriminate this from non-infectious triggers of an uncontrollable systemic inflammatory response, so that appropriate use of antimicrobials and/or other treatments can be initiated ASAP to improve outcomes. However, it is also important to realize that cumulative clinical research evidence suggests that both infectious and non-infectious diseases that lead to hospitalization with SIRS, "progress in a very similar manner". Therefore, a common treatment can very likely be used ASAP to help control SIRS in the early stages, regardless of the triggering event, to minimize greater tissue injury and organ dysfunction. A major contributor to SIRS damage is persistently elevated hyperactivated neutrophils which are not self-sufficiently cleared by macrophage-mediated phagocytosis. As such, these WBCs overreact releasing their chromatin extracellularly (known as extracellular traps) which can increase the risk of hypercoagulation of the blood, glycocalyx-endothelial vascular injury, shock, and DIC. Neutrophils and the Systemic Inflammatory Response Syndrome (SIRS) https://lnkd.in/ePE-tk7b reviewed Sobczak et. al Identification of neutrophil extracellular traps in the blood of patients with systemic inflammatory response syndrome https://lnkd.in/e5XxH4qM Tomono et. al. 2013 Therefore, it seems imperative to help patients as soon as possible, once SIRS is first recognized and immediately put the patient on an extracorporeal whole blood purification therapy capable of helping patients remove potential pathogens and elevated levels of extracellular nuclear chromatin circulating in their blood that has a high affinity for heparin/heparan sulfate. Here is a therapy capable of doing this while a clinician is awaiting the results of other diagnostics to appropriately treat with antimicrobials and/or other drug treatments that may or may not be necessary in helping the patient recover beyond the use of drug-free covalently immobilized heparin blood purification. https://lnkd.in/gg4dzRB6.
