A recent study published in the Annals of Family Medicine revealed that telehealth played a crucial role in offsetting the 17% decrease in in-person primary care visits during the COVID-19 pandemic. However, disparities in overall visit volume were noted across different populations, with significant decreases seen among pediatric patients (24%), Asian patients (11%), and patients with comorbidities (9%) compared to other groups. These findings highlight the need to reimagine primary care delivery—both virtual and in-person—ensuring that everyone has equitable access to care. Primary care is essential for promoting long-term health outcomes and overall well-being. Regular annual checkups with a primary care physician are vital for maintaining health, and telehealth options can enhance access to care. Missed outpatient appointments, or Did Not Attend (DNAs), pose significant challenges for healthcare systems, leading to inefficiencies, increased costs, and exacerbated health inequalities. By leveraging patient waiting list information, English indices of deprivation, and customer segmentation data, we can develop targeted strategies to improve attendance rates and achieve several positive outcomes. Combining these datasets allows us to identify high-risk groups and tailor interventions accordingly. For instance, analysis from NHS Dumfries and Galloway highlights that DNA rates are higher among males, younger adults, and those living in more deprived areas. Understanding these patterns enables specific tailored measures to be applied, such as: 1. Personalised Reminder Systems: Utilising automated reminders tailored to the patient's demographic and socioeconomic background can significantly reduce DNAs. This includes patient app, SMS, email, and phone reminders timed to suit patients' schedules. 2. Flexible Scheduling: Offering appointment times that accommodate the varied schedules of different demographic groups, particularly those with complex social and economic circumstances. 3. Transportation Assistance: Providing transport solutions for patients from deprived areas can remove a significant barrier to attendance. 4. Enhanced Communication: Clear and culturally sensitive communication can address language barriers and improve the perceived value of attending appointments. 5. Support Services: Offering additional support for patients with mental health issues or substance abuse problems, who are shown to have higher DNA rates, can improve their engagement with healthcare services. These strategies not only reduce missed appointments but also contribute to: 1. Improved Healthcare Equality: Ensuring that all patient groups have equitable access to healthcare services. 2. Reduced Administrative and Theatre Costs: Minimising the financial burden on healthcare systems by decreasing the number of missed appointments. 3. Enhanced Patient Outcomes: Timely medical interventions lead to better health outcomes and patient satisfaction. Hartford Foundation philanthropy, the Age-Friendly System-Wide Spread Collaborative will see participating organizations sharing data and other learnings to scale equitable care delivery for the growing demographic. A Top 4 Global Health Equity Solution and Top 15 Global Remote Monitoring Company, AccendoWave, benchmarks objective brain wave pain data (specialty, gender, age) and has nine #pain databases: Emergency Department, Maternal Health, Oncology, MSK, Medical Surgical, ICU, hashtag #Women, Adults, Seniors to eliminate bias, improve outcomes and reduce health care costs. Patient Medication Adherence. This issue is estimated to cost $100-$300 billion and result in 125 000 deaths every year. This is not a new challenge—apparently it appears on Hippocrates’ ancient scrolls. But it’s as serious an issue as ever, 𝐞𝐬𝐩𝐞𝐜𝐢𝐚𝐥𝐥𝐲 𝐢𝐧 𝐭𝐡𝐞 𝐚𝐠𝐢𝐧𝐠 𝐩𝐨𝐩𝐮𝐥𝐚𝐭𝐢𝐨𝐧. Patient adherence to medication regimens is estimated around 50-60%, and there is a huge drop-off in adherence after the first month of a new medication or treatment regimen. #aging #seniorcare #adherence #concordance
