Chronic Low Back Pain: Global Incidence, Prevalence, Symptoms, and Management Strategies **Introduction** Chronic low back pain (CLBP) remains a significant public health issue worldwide, affecting the global population indiscriminately. It is a leading cause of disability, impacting not only the physical well-being of individuals but also their psychological health and socio-economic status. In this lecture, we will explore the incidence and prevalence of CLBP globally, its common symptoms, and the comprehensive management strategies that can be adopted by healthcare professionals. **Global Incidence and Prevalence** Chronic low back pain is defined as pain that persists for 12 weeks or longer, even after an initial injury or underlying cause of acute low back pain has been treated. The incidence of CLBP varies considerably across different regions due to factors such as population age, lifestyle, economic activities, and healthcare access. Globally, it is estimated that about 23% of the world's population experiences low back pain at any given time, with a lifetime prevalence in the range of 60% to 70% in industrialized countries. The incidence of new cases per year is approximately 5% of the adult population. **Symptoms of Chronic Low Back Pain** The symptoms of CLBP can vary significantly from person to person but typically include: - **Persistent pain** that doesn’t go away and might be described as aching, dull, or throbbing. - **Stiffness** in the lower back area, restricting range of motion. - **Muscle spasms** which can be severe and cause significant discomfort. - **Pain that radiates** to the buttocks or legs, indicating involvement of the sciatic nerve, commonly known as sciatica. - **Difficulty in moving** that can be severe enough to impede daily activities like walking, standing, or sitting for prolonged periods. **Management Strategies** The management of CLBP is multifaceted and should be tailored to the individual patient’s symptoms, physical condition, and specific needs. Here are some common strategies: 1. **Education and Self-management**: Educating patients about the nature of chronic low back pain and encouraging self-management strategies such as active lifestyle modifications and regular exercise. 2. **Physical Therapy**: Physiotherapy plays a crucial role in the management of CLBP. Techniques may include manual therapy, exercises to improve mobility and strengthen the core of back muscles, and modalities like heat pack,stretching, message However recent research evidence suggest, Lumbar traction, Ultrasound thareapy and IFT has no effect on chronic low back pain. 3. **Pharmacological Treatments**: Medication, including non-steroidal anti-inflammatory drugs (NSAIDs), and in some cases, opioids. 4. **Psychological Therapies**: Because chronic pain can be related to psychological factors, therapies such as cognitive behavioral therapy (CBT) can be effective in managing the psychological aspects.
