Please share and sign our petition for an Independent Patient Safety Commissioner. Unsafe staffing levels, overwhelmed hospitals/GP clinics, and the inability of all of our health entities to identify, mitigate and properly monitor harm, means safety cannot be assured. This role essentially would be bringing something new to our health system- independent scrutiny which is desperately needed. The PSC would be a proactive force, someone at the helm ensuring that there is no siloing of information within existing agencies, and that there is a timely, cohesive and concerted response to evidence and patterns of harm. The PSC would hold a mirror up to the health system to focus on reducing the harm that is happening, and in the long term hopefully reduce the number of complaints to the HDC and treatment injury claims to ACC. # We need a Patient Safety Commissioner so that no New Zealander, no health consumer has to fight to prevent harm in the health care system. Last week, the Governance team at Sussex Premier Health hosted a Governance Awareness Week, aimed at raising awareness of governance practices in the workplace. The week kicked off with an exploration of the Seven Pillars of Governance, encouraging staff to reflect on what each pillar meant to them within their respective roles. Earlier this week, a report on the Care Quality Commission's operational effectiveness was published, leading Health Secretary Wes Streeting to call for an overhaul of not just the CQC, but all other bodies responsible for patient safety in England. Led by Dr Penny Dash, the report highlights the CQC's operational failings, which include a lack of inspections taking place, problematic IT systems, and inconsistencies in assessments. Our article provides a summary of Dr. Dash's findings and recommendations, with a particular focus on the criticisms of the CQC’s single assessment framework and the need for reform.
