Bariatric surgery is proven as an effective treatment for obesity associated with long term weight loss and a reduction in obesity related comorbidities [1]. The NICE guidance highlights the importance of follow-up post-surgery [2]. I am proud to share that as a member of the APSA Outcomes and Evidence-based Practice Committee, we have published a significant systematic review and evidence-based guideline in the Journal of Pediatric Surgery! Title: Classification and Surgical Management of Anorectal Malformations: A Systematic Review and Evidence-based Guideline from the APSA Outcomes and Evidence-based Practice Committee 🔍 Highlights: • Treatment of neonates with anorectal malformations (ARMs) is challenging due to variability in definitions, surgical approaches, and outcomes. • Our review summarizes existing evidence, identifies treatment controversies, and provides guidelines for perioperative care. • We reviewed 10,843 publications, with 90 included in our final recommendations. • Both loop and divided colostomies are viable options, though loop colostomies have higher prolapse rates. • No significant differences in outcomes between early and late repair groups. • Clear and uniform definitions are essential for future comparisons and recommendations. 📈 Conclusion: Evidence-based best practices for ARMs are still lacking in many areas. Multi-institutional registries and further prospective studies are crucial to improve care and develop consensus guidelines for this complex patient population. She has a history of intermittent vomiting and poor weight gain since birth, diagnosed and treated as gastro-esophageal reflux disease. Exam shows a deyhdrated, but non-toxic baby weighing 5.4 kg (15th percentile). Her abdomen is soft and non-tender. This narrative #review summarizes the current evidence for left-sided uncomplicated and complicated diverticulitis. The latest pathophysiology, advances in diagnosis, and prevention strategies are also reviewed. Observations Treatment is moving to the outpatient setting, physicians are forgoing antibiotics for uncomplicated disease, and the decision for elective surgery for diverticulitis has become preference sensitive. Furthermore, the most current data guiding surgical management of diverticulitis include the adoption of new minimally invasive and robot-assisted techniques. Conclusions and Relevance This review provides an updated summary of the best practices in the management of diverticulitis to guide #colorectal and general surgeons in their treatment of patients with this common disease.

BBC Newsnight recently covered our research into the impact of the outsourcing of NHS cataract surgery on the finances of the health service. In addition to showing a very rapid increase in the number of cataract operations done in the private sector we also found an increasing share of the NHS eye care budget going to cataracts at the expense of more complex conditions such as glaucoma which lead to irreversible sight loss. The rapid growth in the number of NHS cataracts being delivered might be the result of "supplier induced demand" - over the past 5 years 78 new, low cost private clinics have been set up across England and have arguably driven demand for catarct care. There has also been a very sharp increase in the number of complex cataracts funded by the NHS - almost all of the increase in complex cataracts has taken place in the private sector over the past 5 years. According to NHS England, this increase cannot be explained by changes in the complexity of patients being treated. Providers can charge an extra £400 for a complex cataract and so there is a financial incentive to "code" a patient as more complex. We estimate that if the private sector was coding its cataracts in the same way as the NHS - ie it was generating the same proportion of its income from complex cataracts as the NHS - the taxpayer would have saved £29 million over the past 2 years. 📢 Exciting new research published in the Annals of Surgery presents a practical strategy to reduce surgical overtreatment of intraductal papillary mucinous neoplasms (IPMNs) of the pancreas! 🧪🔬 This groundbreaking study utilized a Markov model analysis to evaluate different management strategies for IPMNs and their impact on patient outcomes. The findings highlight the crucial role of age in decision-making, shedding light on how we can better tailor treatments and reduce unnecessary surgeries. 💡💉 🔎 Key Findings: ✅ Consensus guidelines for IPMN management have limitations due to a lack of relevant data. ✅ Age is a critical factor in determining the risk of malignant conversion and surgical mortality. ✅ Prophylactic resection showed a net gain in patient-years for low-risk IPMNs up to age 60, while high-risk IPMNs saw a net gain until age 75. ✅ Surgeons should consider a comprehensive approach, taking into account clinical variables, patient expectations, preferences, beliefs, and comorbidities. This research provides valuable insights into improving the management of IPMNs and reducing surgical overtreatment.
