🚀 The CoreTherm® concept: a game changer in BPH treatment Have you ever wondered about the science behind today's minimally invasive temperature feedback microwave thermotherapy for Benign Prostatic Enlargement (BPE)? Let me take you through the journey of how we developed the CoreTherm Concept®. 🔬 In 1997, I pioneered intraprostatic temperature monitoring and feedback technology to individualize Transurethral Microwave Thermotherapy (TUMT) for each patient. By collaborating with Professor Persson in Lund, we applied Pennes’ bioheat equation and Henriques’ tissue damage model to translate temperature, energy, and time factors into meaningful clinical data. This breakthrough allowed us to monitor intraprostatic blood flow and the accumulated “cell kill” in real time. We discovered that during TUMT, blood flow increased by up to 100%, preventing heat build-up inside the prostate. Yet, in about 20% of patients, high blood flow caused treatment failures, which we identified using transrectal color Doppler ultrasound. 💉 In 2003, I developed the Schelin Catheter®, revolutionizing the procedure by enabling sterile administration of local anesthesia with adrenaline directly into the prostate. This innovation transformed a painful, one-hour treatment into an 8-minute, pain-free procedure. 🧪 In 2004, crucial studies in Minneapolis validated our biophysical equations, proving the relationship between time, temperature, and cell death. These findings were key to making the CoreTherm® software a reliable tool for real-time treatment feedback. 🌟 In 2006, I introduced the CoreFlow Soft Stent®, a temporary stent that replaces indwelling catheters after TUMT, reducing discomfort and enabling immediate spontaneous micturition. It’s also a diagnostic tool for patients with neurological diseases, allowing us to assess bladder function before making treatment decisions. ⚡ My fourth innovation in 2010—the “Secondary Treatment Endpoint”—marked a major leap forward, enabling treatments to be completed based on energy dosage relative to prostate volume, thus minimizing the risk of under- or overtreatment. The culmination of these innovations is the CoreTherm Eagle®, the next-generation machine that applies all these principles for a safe and effective outpatient procedure.
