This week, Orlando Health secured the approval of a bankruptcy court judge to acquire three Florida hospitals from Steward Health Care. Orlando Health plans to take over three hospitals on Florida's Space Coast: Rockledge Regional Medical Center, Melbourne Regional Medical Center, and Sebastian River Medical Center. The system is pledging to deliver a new era of patient care in those communities.

It is a sad testament to the state of healthcare in New Yorks State that there are more F rates facilities in New York than anywhere else, and New York does not boast one single A rated facility. I fear the situation will only worsen especially with recent reports that there are more rural hospitals at risk of closure in New York than any other state. Nashoba Valley Medical Center faces potential closure on August 31, raising significant concern among local communities and first responders. The closure looms as Steward Health Care, the hospital's owner, has filed for Chapter 11 bankruptcy and is transitioning six of its hospitals to new ownership. Unfortunately, Nashoba Valley Medical Center has no buyer yet. This situation violates Massachusetts state law requiring a 120-day notice before closing a hospital, according to the Massachusetts Nurses Association (MNA). The MNA has expressed worries about the broader impact on healthcare services, noting that other hospitals, already overwhelmed, will have to accommodate displaced patients. Local emergency response times are expected to nearly double, affecting towns like Ayer, Pepperell, and Groton, with patients being transported to hospitals in Leominster, Lowell, Concord, or Nashua, NH. Lawmakers, including Rep. Ayanna Pressley and Sen. Elizabeth Warren, have criticized Steward Health Care's leadership and demanded accountability, while a Senate committee investigates Steward’s financial practices. Further visability as to the why! Potentially selling Shields after three years is indicative Walgreens just doesn’t get the services business. On top of exiting a rather dense series of markets as Tampa and Orlando also suggests they didn’t get the VBC components as well. You would have thought as they acquired these businesses that they would have retained the original management teams to insure not only a suitable transition but a sustainable model for success. The fact that the executive team is out suggests they blew it badly. It’s a rather impressive failure of a once terrifically performing business. I suspect they will get it resolved but at the expense of billions to its shareholders is somewhat disappointing and extraordinary. I learned this lesson early in my career that when acquiring a new business especially in the services sector make sure to retain the founding management team as everything is anchored there. Brand, relationships, know-how, customers, and just about everything else. Failure to do so will lead to these types of catastrophic outcomes.

Once again, our staff at Cheshire Medical Center is trying to triage a major public healthcare problem. However, this time, it’s not a global pandemic prompting new patient pipeline issues. Instead, it’s about battling something perhaps even more complex — an unprecedented overcrowding situation. Harneet Sethi, MD, chair of Cheshire Medical Center's Emergency Department, explained the nuanced problem in a recent op-ed that appeared in The Keene Sentinel. If you or a loved one has recently required care in the emergency department anywhere in New Hampshire, then you are probably aware of the longer-than-normal wait times. So, the most obvious question — why the wait? Unfortunately, there are no easy answers or quick fixes to this healthcare conundrum. As someone who has dedicated my professional life to serving the urgent medical needs of the Keene community and our surrounding area, I’m compelled to be transparent with you (or residents) about this mounting public health quandary. By making our community members more aware of the nuanced issues surrounding this problem, hopefully, we can work together to correct it. At the most basic level, the overcrowding crisis is about numbers — too many patients, and not enough beds. Currently, Cheshire Medical Center is operating with two-thirds of the inpatient beds unavailable due to “boarding” of both behavioral health and medical patients. If the legislature can be extended and enough use cases created to prove the concept so that all Medicare Beneficiaries have access to the "hospital-at-home" model beyond 2027, then not only will the cost of care tremendously decrease at hospitals, but it will provide significant solutions to overcrowded hospitals, increase reach and access to rural areas, decrease hospital readmissions, and improve patient's overall quality of life. Yes, there's still a lot to figure out, primarily the billing concepts, as this is not directly a traditional Medicare Part A Service; perhaps the MLTCs can pick up on this? As someone who had spent much time caring for an ailing family member in their final transition, I have seen tremendous benefits of this concept - primarily making their final time easier, kinder, and more humane - and it gave our family something that can never be taken for granted, seeing more smiles and peace on the family member's face when they were at home - rather than the agony, fear, contempt, and delirium when they were in the hospital.
