Clinicians may struggle to recognize Giggle Incontinence (GI) issues in children and preadolescents due to a lack of disclosure, often caused by embarrassment. One such issue is giggle incontinence, which can be difficult to treat. However, first-line treatment options include reassurance of cure, timed voiding or bowel management, and biofeedback and Kegel exercises, regardless of GI severity. Second-line treatment with oxybutynin and timed voiding could be tried, followed by third-line treatment with MPH, though this option may be objectionable to some parents due to its association with attention deficit hyperactivity disorder. Risk-based agreements in the pediatric space are not perfect. They need a lot of work to ensure that we are truly compensating pediatricians for the value they deliver. But that doesn't mean that we shouldn't pursue risk-based contracts, collaborate to make them better, and advocate for them to strengthen how pediatrics is delivered in this country. I struggle when I'm in conversations where people say it can't (or won't) work because it isn't fully solutioned or figured out. Let's work together to figure it out. Dr. Wood from Children's Hospital Los Angeles (CHLA) highlighted that pediatricians could be missing signs of long COVID in children due to lack of awareness. 👶 Rare disease spotlight: Thanatophoric Dysplasia 👶 A severe skeletal disorder characterized by a disproportionately small ribcage, extremely short limbs and folds of extra skin on the arms and legs. Infants with this condition have disproportionately short arms and legs with extra folds of skin. Other signs of the disorder include a narrow chest, small ribs, underdeveloped lungs, and an enlarged head with a large forehead and prominent, wide-spaced eyes.
