🏅 Don't Forget Your Skittles - I write about why it's important to celebrate our accomplishments, recalling an interaction with one of my daughters when she was much younger. It's a 2 min read. Here's some brief context As we get older, our lives become busier and more complicated. There's more to do. There's more at stake. There's more to balance. We're always trying to catch up, and sometimes we're simply trying to be a little less far behind. Time becomes a resource to be optimized or efficiently used; checklists serve as our guide. We finish one shift, paper, or project, and move on to the next. After all, there's so much more to be done. Imagine a world where premature babies, born as early as 22 weeks, can safely continue their development outside the womb 🚼 Researchers at the Children’s Hospital of Philadelphia are working on an artificial womb, dubbed “Extend,” designed to help these tiny warriors survive and thrive. Extend and similar groundbreaking technologies aim to replicate the natural environment of the womb, giving premature infants the chance to develop their lungs and other vital organs more fully. While the science is promising, it's important to keep in mind the ethical considerations. The transition from experimental trials to real-world application involves complex decisions about parental consent, potential risks and future implications for neonatal care. Our publication in JPeds. In preterm infants born at 23-25 weeks, noninvasive respiratory support during the initial stabilization in the first 30 min after birth, compared to intubation, is associated with lower odds of death or severe brain injury before discharge from NICU after adjustment for confouders. This is despite the fact that 77% of babies received noninvasive respiratory support were intubated within 7 days. Although, there was no significant difference between noninvasive respiratory support and intubation groups in neurodevelopmental outcome at 18-24 months, all point estimates were below 1. Another evidence supporting the use of noninvasive respiratory support instead of intubation in stable micro preterm infants.

I think it is with MUCH CAUTION TO LEAD WITH THIS TITLE: Painful NICU procedures change neurologic development in Preterm babies We have made tremendous steps forward in being able to have such incredible survival in preterm deliveries which of course is not without other long term risk and ongoing evaluation to minimize that risk However, even in the paper itself the statement is "While not directly causative, we may be observing early disruptions in this network’s formation with co-activations through repeated painful exposures" This is CORRELATIVE data... NOT CAUSATIVE And it is our duty to make sure the message is shared appropriately and continue to question things like... Were more procedures performed in children who were higher medical complexity and that's why there were more skin breaks? Was the reason for prematurity a contributing factor? Being more premature means more time and more need for more pricks... (The scatter plots also suggest that the there may be an effect just based on being extremely premature and more susceptible to more skin breaks) This is not intended to minimize this amazing work accomplished... This is a great study and should be applauded! Just emphasizing significant caution on how to interpret and implications on practice... Yes, be mindful if you need those labs but don't skimp on labs or treatments needed if it what's necessary...
