#CMOs need their physicians to be addressing prescription overload. Why? Not only does it affect patient care and quality, but because medication management is a component of #valuebasedcare, according to Karna Patel MD, MPH, vice president at Tampa General Hospital and president of Tampa General Medical Group. "We are focused more on patient experience, quality, outcomes, and cost of care," Patel says. "Medication management is an important area for CMOs and health systems because if we are aiming for all of those things, we want to make sure patients are getting the medications that are needed—not multiple medications that are duplicated or are interacting." There are other reasons CMOs should be concerned about patients with multiple medications, he told us. "The side effects that can result from multiple medications can lead to increases in #healthcareutilization, risk of going to the emergency department, hospital admissions, or ICU stays," Patel says. According to the National Coordinating Council for Medication Error Reporting and Prevention (NCCMERP), medication errors are categorized into different levels based on their potential harm to patients. 🔍 Categories of Medication Errors by NCCMERP: 1️⃣ Category A: Potential errors that didn't reach or harm the patient. 2️⃣ Category B: Errors are caught before they reach the patient, so no harm is done. 3️⃣ Category C: Errors reached the patient, needing monitoring to ensure no harm or intervention to prevent it. 4️⃣ Category D: Errors reached the patient, needing more monitoring but didn't cause harm. 5️⃣ Category E: Errors caused temporary harm with no lasting effects. 6️⃣ Category F: Errors caused temporary harm needing hospitalization. 7️⃣ Category G: Errors caused permanent harm. 8️⃣ Category H: Errors needed intervention to sustain life. 9️⃣ Category I: Errors contributed to or resulted in the patient's death. What is a diagnosis-related group (DRG)? A diagnosis-related group (DRG) is a case-mix complexity system implemented to categorize patients with similar clinical diagnoses in order to better control hospital costs and determine payor reimbursement rates. For example, Medicare pays out a set amount based on a patient’s DRG as opposed to reimbursing the hospital for its total costs. This method encourages the hospital to minimize care costs. in other words DRG for appendicitis for all patients is equal Except in special situations when severity increase by other complications in this case price increase next time will know how to calculate it.

MBI welcomes the findings of the report The Hidden Waitlist: The Growing Follow-up Backlog, published today by Reform Think Tank, Sebastian Rees, Hashmath Hassan, which estimates that the total number of people currently waiting for follow-up appointments at acute Trusts in England stands at more than 11 million. “The report highlights the need for greater transparency of the follow-up backlog. Whilst the total size of the list will get headlines, the real question is: how many of the total are overdue? The gaps in information are concerning, but perhaps inevitable given the polarised focus on one set of patients over another. We fully support the report’s main recommendation to report on these patients regularly. It is only through full transparency of patient demand and visibility of all waiting list activity that hospitals and the NHS can properly inform patient safety, recovery planning and direct precious resource. With patients increasingly suffering negative outcomes from long waits or missed follow-up appointments, it’s essential that the NHS has the data to prioritise all of its patients, not just RTT. We need to understand the competing demands between these lists and focus on patient care and risk. Transparency and better data are the basis for doing this. The report raises some key policy issues in how elective care patients are being managed in different ways depending on which list they are on.
