❕ Secure the airway asap ❕ In a cardiac arrest, intubate ASAP. The ACLS algorithm lists airway management as a critical step - right alongside of CPR and defibrillation. ⏳ Time is critical. A brain 🧠 without oxygen is a dead one. ❔ What can an LNC look for to determine standard of care was met❔ When reviewing a cardiac arrest case, look to see when an airway was secured. Was it within minutes of identifying an arrest or was it much longer? For those taking a while to intubate, ask yourself a few questions: 🗺 Location: pre-hospital vs in-hospital? EMS can take time to show up - especially if it's a rural area. The response in the hospital should be within minutes. 🚑 Resouces: What were the resources available? If an arrest occurred at home or an outpatient center, intubation supplies will not be available until EMS arrives. Did this occur in the ER, on a med-surg unit, or in the ICU? Each may or may not have certain supplies or the anesthesia team immediately handy. Ask ALOT of questions about where things are at. 💡 Knowledge: Who was there and what were their credentials? ALL healthcare workers are responsible for knowing BLS or ACLS - even if they do not work in a hospital setting. Look closely at who responded in order to determine if a knowledge-deficit was present. The 🅰 in ABCs is there for a reason. Secure the airway! Any tips for determining appropriate airway management as an LNC? 🔽 🔽 ------------------------------------------------------------------------------------- If your questioning intubation timing on a post-arrest case, reach out! Explore “Advancing Infusion Therapy Practices: Prioritizing Patient Experience and Safety” with Marlene M. Steinheiser, Infusion Nurses Society, at the 2024 NICA Annual Conference at the MGM Grand, Las Vegas, June 21-22. Register now at bit.ly/NICA2024 Because intravenous (IV) administration of medications and solutions is invasive and risky, it is critical to ensure that clinicians are applying evidence-based best practices as they advocate for safe patient care. One of the most commonly used vascular access devices, the peripheral intravenous catheter (PIVC), can lead to insertion and post-insertion complications. Insertion and management of PIVCs requires knowledge, skill, and clinical judgment to ensure positive patient outcomes, but current literature describes a gap in education, lack of consistent evidence-based practice, lack of adherence to policies and procedures, and the need for data collection to improve care of patients with PIVCs. In this session, we will discuss current INS Standards related to PIVCs, including the basics of PIVC selection, insertion, care, and management, learn from patient case studies, hear a patient’s story of self-advocacy, describe how the development of a PIVC consensus statement and health equity research can benefit patient outcomes and how to begin your own quality improvement project. #infusionprovider #medicalprovider #infusion #infusioncenter #infusionnurse #infusiondeliverychannel #medicalinfusionprovider

Advancing nursing research at Middlesex Health! Check out my new publication. In our TIMING study, we found that when our cardiac rehab patients exercise nonconcordantly to their circadian rhythm (CLOCK SNP rs1801260) there was a significant increase in their ED utilization and readmission rate at 90 days for ACS patients. TT genotypes perform physically better in the morning than the C allele (TC/CC) and thus may benefit from morning cardiac rehab. (Molle, E., Lee, L., Rzecka, J., & Pallas, M. (2023). Time of Day Effects on Exercising in Cardiac Rehabilitation. Journal of Cardiopulmonary Rehabilitation and Prevention, 43(6), E22-E23).

when patients got delayed to perform MRI/CT 1) Immediately acknowledge the delay and apologize for any inconvenience caused to the patient. 2) Promise to expedite the report, aiming to deliver it on the same day or within 3 to 4 hours. 3) If the patient has other scheduled scans/tests, prioritize completing those or request to proceed with them to minimize overall wait time. 4) Politely explain the reason for the delay, mentioning the need to prioritize emergency cases such as ER patients, ICU cases, strokes, or chest PE. 5) If the patient agrees, offer to reschedule their appointment to a more convenient time. 6)Try to calm the patient, taking help from a nurse or Patient Relations Executive (PRE) if necessary. 7) Keep the patient informed with regular updates about the expected wait time and any changes. 8) If possible, offer comfort measures such as a more comfortable waiting area, refreshments, or reading materials. 9) Ensure that any rescheduled appointment is followed up promptly, confirming the new appointment details with the patient. 10) Document the reason for the delay and any actions taken please add if anything missing
