Analysis of more than 1,600 vessel inspection reports in year to 1 April 2024 reveals ongoing safety concerns over confined spaces. Our analysis found that in the last year: 📋 7% did not have a technical inspection by the vessel operator. ⛔ 9% did not adequately control enclosed space entry. 👷♂️ 12% did not inspect the pilot ladder before use. 🔥 10% did not have procedures on the handling of chemicals and flammable materials. 🖥 11% did not have cybersecurity measures in place. ⚕ 5% did not carry sufficient medical supplies on board. Mark Ford, Marine and Quality Manager at IMCA said “From the beginning of all seafarers’ basic training, we are instructed on the hazards and procedures around confined spaces, yet sadly we are continuing to hear of unnecessary fatalities. No one should lose their life doing their job, however more than 300 are known to have died in enclosed spaces on ships since 1996, including 31 in 2023.” The 53-page report (IMCA M267) is freely available to support the improvement of our sector. While experience is paramount in this industry, you can also learn a lot from other people’s experience. Each year the National Transportation Safety Board (NTSB) releases incident and accident reports and each year there is at least one instance where the probable cause to these (often deadly) incidents is “insufficient lookout.” Maintaining a proper watch is not just for vessels that lack AIS or Radar, it’s a legal mandate for all vessels underway, and for good reason.

Here's a summary of major changes: • Changes the title from “Safety and Health Requirements Manual” to “Safety and Occupational Health Requirements”. • Incorporates a new U.S. Corps of Engineers Safety and Occupational Health Management System (CE-SOHMS). • Eliminates the term Government Designated Authority (GDA) within this manual. • Streamlines document to reduce redundancies and eliminate unsubstantiated requirements. • Adds new requirements for Site Safety and Health Officers. • Adds requirements for fire prevention and protection associated with fire watches. • Adds requirements for vehicles, machinery, and mechanized equipment associated with defensive driving. • Adds requirements for loading handling equipment medical evaluation. • Adds requirements for blasting associated with the Blaster-in-Charge and explosive site safety plan. • Adds requirements for sanitation associated with food service training and extermination plan. • Adds new requirements for uncrewed aircraft. • Incorporates requirements for the Army Electrical and Hazardous Energy Control (HEC) Safety Program.
