*WOW* I just completed reading all 169 pages of the CSB's incident report into the fatal Naptha release from BP-Husky's Toledo refinery on 20th September 2022. 6 hours of almost continuous face palming 🤦♂️ (I was on a flight) ... This report is extraordinary, and I strongly recommend all Process, Automation / Control engineers, as well as facility operators and supervisors read it (link below). Two brothers were killed while trying to recover an unrecoverable cascade of failures and overflowing vessels. The report outlines multiple levels of failure, including failure to learn from previous incidents (including Texas city), failure to implement stop work authority, failure of MOC on multiple levels, failure to implement proper controls and a poor safety culture where production was king. Alarm floods, poor instrumentation/controls and confusion reigned supreme. I'm honestly flabbergasted. Please read, and learn from it. I'll be preparing a companywide safety presentation for Wood on this, and hope everyone in the industry does so as well to ensure such a tragedy is not repeated.
