We are fast approaching the end of MS awareness week and I want to share a personal story I have witnessed. Day 1: Ophthalmology clinic - 'Hmmm, can you come back in the afternoon - I'm not sure what this is?' (favour as works in the NHS) Day 1: Consultant ophthalmologist PM clinic - 'This might be optic neuritis. You know this may be the first sign on MS - come back in 6 weeks and we'll see what your vision is like' Day 2: Consultant neuro-opthalmologist (favour second opinion another centre) - 'This is optic neuritis. Let get an MRI head and spine in 6 months to see if its MS' Day 3: Urgent MRI head and spine (favour another centre) Day 3: Consultant MS neurologist (favour) - 'Yes, this is MS. You have mild symptoms, no need for treatment. We'll just keep an eye on how you're getting along. You'll be fine.' Day 5: MS national expert (through favours) - 'You have had 3 other early warning signs of MS over the last 3 years. It is wise we commence you on treatment - there are options such as xyz. The evidence suggests xyz. Side effects are xyz. Of relevance to you specifically given your life and background is xyz. We'll discuss at our meeting and get you treated soon.' 1 month later: Treatment commenced 5 years later: Stable illness This person was lucky - their journey was far from perfect but they were still lucky They had inside access and favours they could ask for to ensure they were diagnosed and treated early and extremely well by top experts. 🛑This is not the case for the vast majority of people🛑 IT SHOULD BE. There should not be a discrepancy in quality. But there is. In every field. Published April 11, the study tracked 1,470 adults with chronic low back pain over the course of 12 months. Patients who were treated by “very empathic” physicians and patients who were treated by “slightly empathic” physicians both reported on outcomes relating to pain levels, function, and health-related quality of life. The findings — which also compared physician empathy to treatments like opioid therapy and even surgery — are compelling. Today we’ve published the latest National Ophthalmology Audit (NOD) age-related macular degeneration (AMD) audit report. Read it here https://ow.ly/cwrE50R0X0o Thanks to everyone who contributed. We’re encouraging more ophthalmology services to take part in future audits so that more data on the care pathway is available and treatment outcomes can be standardised. Key findings include: • Treatment was very effective in preserving and, to some extent, improving vision. • The best outcomes were observed in younger patients, in eyes with good baseline acuity and when the initial phase of monthly treatment was not delayed. • Treatment of NvAMD requires considerable commitment of time and resources by both patients and treating centres. • On average, more than two-thirds of injections were administered by nurses or other trained healthcare professionals rather than doctors.
