If ever there was a case for universal health care in Australia it is to deliver equitable care to residents of Aged Care Facilities. Our federated funding model ,lack of interoperability , geographical isolation and more often than not an inability to advocate for themselves means they are left to the mercy of old systems , small business and varying quality of private provider models. Access to primary care is varied as is how those encounters are recorded . So how do we ever get a true picture of who is getting care and who isn’t and what kind of care they are getting. This isn’t good enough. We know the outcome of this model of care. It is all detailed in the Aged Care Royal Commission. The idea of 24 hr Nursing Care in aged care facilities is a great one but where is the interoperability that will deliver the connectivity with hospitals and primary care in near to real time to ensure these nurses are supported and connected to Primary and Secondary care when they need it. Without it I imagine this would be one of the most stressful jobs in healthcare as residents most likely have comorbidity , polypharmacy cognitive decline and mobility issues to manage so this nurse is going to need a great model of care and a connected system. #modelofcare #connectedcare Health Services Daily
