The agreement between the plasma clearance of relmapirazin and that of iohexol is impressive (10.1016/j.kint.2024.06.012). The agreement between the plasma clearance of iohexol and that of iothalamate, by contrast, was much poorer (10.1053/d.ajkd.2016.01.007).  And the agreement between 51CrEDTA and 99mTcDTPA, whose assay precision is by definition excellent (gamma counting), also appears to be poorer (10.1007/s40620-020-00932-9). In my opinion, the very good agreement between the plasma clearances of relmapirazin and iohexol has several explanations:  - the measurements were simultaneous,  - by an identical method (plasma clearance vs plasma clearance, no urinary clearance). - And maybe the relmapirazin assay was ‘calibrated’ to be close to that of iohexol, even though these are molecules of different molecular weights whose percentage of tracer excreted in the urine seems slightly discordant with low GFRs. External validation is essential, as is comparison of the plasma clearance of relmapirazin with other exogenous tracers. Assessment of the urinary clearance of this new tracer is also essential, as it is the only valid method of measuring GFR in cases of volume overload (which is common in acute and chronic renal disease).
