The "require over 25 direct practice hours per week" is A HUGE ONE for me. If you've been following me for any amount of time, you'll know I'm a tremendous advocate for lower and more manageable caseloads 👏 Some of the most common refrains or rebuttals I hear to this is either some form of saying that clinics won't make money if they don't require higher caseloads OR some version of saying that clinicians somehow don't deserve to make a living wage if obscene caseload numbers of 25-30+ per week aren't sustainable for them. What I have to say about that and what I know Megan Cornish, LICSW and others have talked about before is that if you can't afford to pay your therapists a wage that backs the high level of education, training, and expertise they possess without requiring sky high productivity numbers, then you don't actually have a functional, sustainable business model. I know that I personally will never, ever work for a clinic or practice again that requires upwards of 24-25+ billable clinical hours per week, and I know that's a loss for them 🤷♂️

In my recent work at PrescribeLife I encountered yet another sad and profound moment with a founder facing the darkest side of startup life: an episode of self-harm. This experience is an unfortunate and timely reminder of the often unseen issues that leaders of startups experience. Just yesterday, a new survey (posted by Amy Lewin at Sifted - thank you) highlighted once again the poor state of founder mental health. As a psychiatrist, a medical expert in the field of mental health medicine, I am used to dealing with self-harm situations within a hospital setting. However, now seeing this frequently occurring in the often superficially glossy world of startups is especially unsettling. Startup life is frequently celebrated for its highs, but we seldom discuss the lows with the same openness. The pressure to succeed, fear of failure, and constant uncertainty can weigh heavily on mental and physical health. To any struggling founders, know you're not alone.
