😞Shame is the uglier side of medical training and the intended maintenance of inhumane control of conscientious physicians by authoritarian organizations. Physicians are primed in medical school with shame. It can then be weaponized later in their careers for the sole purpose of power and control. 😢It is dehumanizing and abusive. It results in demoralization, depersonalization, and eventually burnout. It is one of the more common causes of suicide— which is already high in the medical profession. It is more likely to adversely effect minorities including racialised, LBTQ, and neurodivergent physicians. 🌈Medical organizational culture from medical school to senior practice needs to eradicate shaming as a mind control tool. Sinister manipulation of conscientiousness is abusive. There is no place for shaming innocent people in any healthcare culture. During Pride Month, let’s protest the weaponising of shame. #medicalculture

This podcast episode takes a sobering look at the mental health of U.S. medical students and trainees, in an era when the pressures can be intolerable. However, the culture still treats depression as a weakness. This post and episode discuss mental illness and suicide. And so I want to warn listeners at the outset that some of this content may be distressing. The United States has a high rate of physician suicide, with 400 physicians taking their own lives each year. According to AAMC, almost 30% of medical students and residents suffer from depression, and 10% report having suicidal thoughts. According to another study, the global prevalence rates of depression and anxiety among medical students are estimated to be 27% and 34%, respectively. Our latest paper on the intersection of trauma and medical care highlighting the critical need for healthcare systems to recognize and address the profound impact of prior trauma on patients experiencing medical stressors. By integrating trauma-informed and trauma-focused practices into psychosocial programming, we can significantly improve patient outcomes. Key insights include: 🔹 The heightened risk of PTSD following medical stressors for trauma-exposed individuals. 🔹 The often-overlooked interaction between prior trauma and current medical experiences. 🔹 The necessity for practical guidance for psychologists to incorporate trauma-focused interventions in medical settings. We emphasize the importance of comprehensive trauma-focused assessment and treatment to enhance care for trauma-exposed patients. This article will be featured as a Clinical Education article in the next print copy of the Journal of Clinical Psychology in Medical Settings. For Jena Roeber, OTR/L, PHS, offering occupational therapy in the hospital after c-section goes beyond physical recovery and function: it’s mental healthcare for mothers after their major, open abdominal surgery. In Omaha, NE, the maternal care team at Nebraska Methodist Health System agrees. “To expect that everyone is going to immediately bounce back from major surgery is ignorant. Toss in the normal cares of an infant into it, hormonal changes and emotional fluctuations, it seems that almost all postpartum moms could benefit from such an innovative approach to postpartum care.” - Dr. Brian Karre, MD, Obstetrician-Gynecologist, Nebraska Methodist Health System Jena Roeber, OTR/L, PHS is trained to recognize postpartum anxiety and depression. “It’s really more common than people think,” Roeber said. “Especially in moms who’ve had unexpected C-sections. They’re dealing with guilt, or maybe they feel like less of a mom because their birth plan didn’t go according to plan. Throw in a traumatic experience on top of that, and it’s all the more important to process some of those thoughts right away, so that when they do go home, they’re in a better state of mind to take care of themselves and Baby.” Offering occupational and physical therapy in the hospital after cesarean section takes a team to recognize that a mother’s recovery is unique to her specific role in taking care of her newborn, other children, managing her own self-care, and facilitating her support system if she is a single parent. Notes: 1. Nebraska Methodist Health System’s Postpartum OT program is designed for patients after c-section with a BMI of 30 or greater. Stay tuned to learn how they expand their services to include more maternal populations and offer physical therapy services in the hospital after birth as well. 2.
