Our latest #ASHAInnovator is Shirley Sigmund, an SLP in California. Discover how Shirley's passion to help patients, clinicians, and providers communicate effectively inspired her to become a CSD professional. What led you to become a CSD professional? "I became a CSD professional because I believe the essence of being human is the ability to communicate. I thought I was going to be a psychologist, but when I was working with a developmentally disabled patient in 1980, I could not understand him, and through context, I figured out what he was asking me. It then struck me that if a patient cannot make his or her ideas or needs known, it would be hard for him or her to be helped. And vice versa, if a provider or clinician cannot understand a patient, the provider will be unable to fully meet his or her needs." Tell us about a time you’ve made an impact in your profession. "I work at a forensic psychiatric hospital, and my main caseload has been diagnosing and treating dysphagia. Through reading the literature and being a keen observer, I realized that many of my patients' new onset dysphagia were side effects from high doses of antipsychotic medications. Some of the psychiatrists were open to hearing my thoughts and made changes in patients' medications which resulted in a significant improvement or resolution of dysphagia. I have presented poster sessions on medication-induced dysphagia, and my handouts are currently used in new employee orientation, so every newly hired physician is made aware of the potential of medication-induced dysphagia." What does being an innovator in the CSD field mean to you? "Being an innovator in the CSD field means collaborating with professionals from the fields of psychology, medicine, computer science, nutrition, education, and engineering (among others) and implementing new technology and approaches that benefit our students, clients, patients, and each other. Did you know that in women, lower scores in specific memory tests correlate with a higher risk of AD? This highlights the importance of tailoring assessments based on gender. Dr Ting Fang Alvin Ang, Data Core Co-lead at FHS-BAP and Dr. Rhoda Au, Director of Neuropsychology for the FHS, emphasize, “By exploring all facets of neuropsychological assessment, we can pave the way for a future where sex-specific predictive models can lead us closer to preventing Alzheimer’s Disease and optimizing cognitive well-being for every individual.” Dr Antonella Chadha Santuccione, pro bono CEO at WBP underscores the significance of these results, stating, “These findings highlight the urgency of understanding not only the pathophysiological difference in Alzheimer’s disease between men and women but most importantly the impact of those on clinical read out measurements such as we can optimise clinical development. Affirming care for the neurodivergent community isn’t optional. It is the bare minimum for adequate healthcare. The neurodiversity-affirming paradigm has only gained momentum in the last two decades. Most healthcare practitioners did not have training in the area and this has had profound implications. Therapeutic and healthcare spaces have not been safe or affirming for neurodivergent individuals. Neurodivergent thinking and functioning is often pathologised by professionals trusted with your health and safety. Neurodivergence is often misdiagnosed, misconceptualized or not identified, which means that people invest time (sometimes decades) and energy into ‘treatment’ that does not support their needs. Neurodivergence does not need to be treated; it needs to be embraced and supported. The trauma attached to living in a world built for neuronormativity needs to be acknowledged and processed. By identifying and acknowledging the systematic problem and the limits of our knowledge and understanding, we can work toward inclusive healthcare experiences. It’s time to raise the bar, and SHIPS is here to be part of that change. An important part of that journey begins with acknowledgement and taking responsibility. We understand that, as healthcare providers, we have been part of and are representative of this system, and we would like to take the opportunity to set the precedent for the change we would like to see. You deserve safety and care, to be valued and to receive adequate support and healthcare.
