“I discussed with the primary team.” One of the most dangerous lines I see documented. As "the primary team," did they discuss with me? No. Why did they document it? Maybe for billing. (So...that's billing fraud.) But they may not know why it's in their template...or even that it's in there at all. If that case were to go to litigation it would be consultant's word versus attending's word. Lawyers LOVE It when doctors are pitted against each other. So. Check your templates. Stop auto populating that dangerous phrase. For yourself and your colleagues.

Are we all doing CAPA wrong? I have a different take on CAPA, instead of wrapping it all up into one process, consider splitting the process. A model that works is Corrective Actions/Preventive actions (CA/PA). Unfortunately most people dread CAPA programs, but if we can spin this and allow the teams with the experience to have some level of ownership over their processes, do companies not benefit? Consider this, if individuals recognize a better way to do things and have a Preventive Action (PA) program to utilize to make these recommendations will companies not experience a drop in the more negative Corrective actions? Now consider providing a financial reward system for suggestions that save the company significant money. How jazzed do you think the team would be to find ways to make improvements? It is human nature to be more excited about their work when we feel like we are contributing more than just punching the clock.
