The Pan American Health Organization (PAHO) is consulting its Member States on the draft WHO Strategy on Traditional, Complementary, and Integrative Medicine (TCIM), gathering input from national health authorities and social actors. 🔖 Recognised since the Alma Ata Declaration in 1978 and reinforced in the Astana Declaration in 2018, TCIM’s role in Primary Health Care (PHC) is crucial. The WHO has previously published two Traditional Medicine Strategies, and the latest draft for 2025-2034, adjusted based on over 200 public comments, is now under review. PAHO and other WHO regional offices will submit consolidated reports for the final draft, to be presented at the 156th WHO Executive Committee session and the 68th World Health Assembly in 2025. When people hear about Value-Based Health Care #VBHC, they often think of measuring outcomes through #PROMs (Patient-Reported Outcomes Measures). But that in itself is NOT SUFFICIENT for creating value from the perspective of patients, and thus, #value for all stakeholders. Integrating care and measuring costs over the whole care pathway are also neccessary. The cost element has garnered lots of attention, but not so much with the integration and the organization of care based on medical condition and patient segments through the use of #IPUs (Integrated Pratice Units). The implmentation of IPUs is only in its early days in North America, but our colleagues in the Netherlands have been applying and improving the model for years. This fantastic article in NEJM Catalyst from one of my favorite thought-leaders and collaborators in VBHC Implementation, Paul Bastiaan van der Nat and collegues, really sums it up well for those looking to optimize health outcomes and costs by organizing IPUs around a medical condition.

Your primary care team will specialise in… you. That was the start of my answer to Steve Gardner questions at Abu Dhabi Healthweek: “What do you see as the role of primary care? Is it a triage tool for secondary care and mental health? Does it exist to create better preventative care, public health and mental health, how is it viewed by the patient population? Payors? Providers? Government?” In my opinion, care will continue to go the most specialised. Specialisation raises outcomes, which crowds out the less specialist, which raises volumes, which raises outcomes again, etc. Digital reinforces this because increased information to the specialist increases their ability to monitor and treat; and information about the specialist spreads so patients choose them. The downsides are that these super specialists know more and more about less and less of you. And they are further and further away from you. What is missing and needed is the local, instant, personalised. Because the closer and faster the care, the less the disease progresses, so the cheaper and better the outcome. That’s primary care - a holistic doctor in your neighbourhood supported by on-demand super specialists and point-of-care AI. So your GP as a specialist must become your localist, your personalist. They specialise in you. It was great to watch Steve sparring with the other panelists Ali Anees (on the left), Mansoor Ahmed (Economist l Strategist) (the other side of me), Walid Kattan and MAHMOUD ALYAMANY MD as we discussed primary care from many different angles. I loved representing Patients Know Best in the event as we have gone past 4 million registered patients and are about to add primary care records functionality for our users across England.

🚨 Healthcare IT: Time for Progress, Not Setbacks 🚨 As a Consultant in Rural Hospiral Medicine, I experience firsthand the frustration of dealing with inadequate IT systems. Every day, I lose at least 30-60 minutes to non-integrated software and printers that need constant reprogramming due to poor IT infrastructure. In a world where technology should be driving efficiency and supporting patient care, we’re often left fighting against systems that seem designed to slow us down. Ian Powell’s recent blog “Health System IT: 0 Steps Forward, Two Steps Back” perfectly captures the reality many of us face. We need to be investing in IT infrastructure, not cutting back. Our patients deserve better, and so do our healthcare professionals. MSK 2024: ‘Health Inequalities: closing the gap’ ‘Labour will tackle the social determinants of health, halving the gap in healthy life expectancy between the richest and poorest regions in England.’ (labour manifesto) Did you know that he biggest commitment in the new government’s health policy is its commitment to tackling health inequalities?.... and it is everybody’s responsibility in the healthcare system to act now to close the gap. This year’s MSK conference is designed for all who have a touchpoint in MSK conditions to support this: clinicians, and therapists, service managers and clinical leads and senior leadership such as ICB leads. The agenda is HUGE and needs everybody to work together to drive change. It is a mixture of keynotes, clinical sessions and workshops to equip all with a toolbox to apply into practice.

Just wrapped up a whole week of lessons, discussions and group work for GMS5153: Non-Communicable Diseases: Building Health Systems Capacity at SingHealth Duke-NUS Global Health Institute! Thanks Prof Wan Cheng Chow and team Natasha Tai Glen Koh, MPH, CHES Orlanda Goh Su-Yen Goh Jack Tan Di D. Lijing L. Yan Anne-Claire Stona, MD, MSc Shreya Agoramurthy Evelyn Wong for the superbly curated course content and for allowing me to be a part of it! This course covered the global burden of NCDs, sparked new ideas on how health systems can be transformed to better integrate NCD care, and offered fresh perspectives that have made me reflect on my work in healthcare. A highlight for me was the dynamic panel discussion with Prof Godelieve Van Heteren, who posed a question that continues to resonate: before we can have just, efficient, and equitable healthcare, we need to ask ourselves whether the society we inhabit is just, efficient, and equitable. It’s a powerful reminder that health systems don’t operate in a vacuum—they are reflections of the societies they serve. The systems we build are only as strong as the foundations they rest on. Admittedly, it is easy to feel overwhelmed by the scale of the challenges. Nevertheless, we need to keep questioning, keep pushing, and most importantly, keep working together 💪💪 #PublicHealth #GlobalHealth #SDGs #NoncommunicableDiseases #HealthEquity #SocialJustice
