This weekend, we did a major paperwork tidy up at home! Whilst sorting through a dusty pile of papers, I came across this article from 2012 featuring my dad. He had just retired as a GP partner, after dedicating over 30 years to a practice in Banbury. His commitment included years of being on-call during evenings and weekends on a demanding schedule of 1-in-5 rotations. Reading his use of the word 'Privilege' to describe his feelings when reflecting on the care he was able to provide his patients through his career, it really struck a chord. Recently, I encountered the same term in a different context. While reading a news article written by a discontented NHS GP, I flipped to the comments section and noted a retired colleague had left a comment emphasising that they should place more focus on how privileged they are to be serving the public as a GP. This sparked introspection.... During my ten years as an NHS GP, I worked both as a partner and a salaried GP: while I took pride in my work, did I truly feel privileged? Certainly, occasional interactions with patients did evoke a sense of privilege. However, overall, the challenges and frustrations of the job overshadowed any generalised feeling of privilege. This ultimately influenced my career trajectory. Now, predominantly practising as a private GP at a practice I started with my wife - also a GP - I find myself genuinely feeling privileged to be a doctor. This may raise eyebrows, but allow me to elaborate. In my current role, I can provide thorough care, treating patients comprehensively and fostering enduring relationships with them and their families. For me, privilege comes not from 'being' a GP, but from the joy of being able to forge meaningful connections, ensure continuity of care, and earn trust regularly on a 1:1 level with my patients. With growing workloads, access challenges, and modern practices using different care models via a more diverse workforce, it is harder than ever for an NHS GP to achieve this at a 1:1 level. If, through recognising and then addressing the currently challenges, the job as a GP can feel a privilege to more doctors, my sense is retention problems will start to go away. While I do encounter NHS GPs who feel the sense of privilege in what they are doing that my dad once felt, they are few and far between, and I tip my hat to them! And, of course, I tip my hat to my incredible dad!

While the job is tireless, it doesn't have to be, nor should it be, thankless. In the emergency department at Northeast Georgia Medical Center Barrow we are trialing all of that at one time. Check out our Press Ganey Superstars board! 🌟This board will be used to recognize staff members that were positively named in our weekly Press Ganey patient experience comments. Each positive comment is shared with staff via email and each positive comment will be symbolized by a gold star ⭐️ placed by the employee’s picture. This creates healthy competition amongst staff, while improving staff recognition and improving patient experience. Additionally, the department has a goal related to the final annual average for patient experience scores. That goal is represented in the middle of the board with my pictures. One which represents our Chick-fil-a 🐥level of customer service and the other which represents poor customer service. If the staff can meet the quarterly goal set then they are rewarded with Chick-fil-a meals 🍗from the management team. Thankful for a Unit Practice Council Chair that helped bring the vision to life! I can’t wait to see all the gold stars on the board. I know this crew will continue to provide top tier (Chick-fil-a level 🐥) customer service and I can’t wait to see the positive impact this has on our patients and their care. You know what? Me too. I read this and felt a deep familiarity. We need primary care doctors actually being primary care doctors. As Guy Culpepper, MD stated, there really isn't any replacement. But can you really blame them? --> Administrative burdens --> Coding risks that come with audits and penalties --> Pressure to see more patients --> Increased Expectations --> Decreased Reimbursement You can't expect a farmer to keep going out in the rain and pelting hail to tend to a crop that's half planted and riddled with disease when someone offers him an umbrella and a job that pays twice as a good in town. Everyone has a solution. Go all in on cash pay. Work for someone who claims to be better. Suck it up. Work harder. I used to be the General Counsel for one of the greatest multi-specialty clinics in the state (Austin Regional Clinic: ARC) and I saw first-hand how these PCPs struggled, adapted, powered their way through, formed strategic alliances, and they are still standing. ARC was formed by Dr. Norman Chenven, an amazing PCP that is still to this day my favorite boss that ever lived. He helped me see what the world of a doctor was like. He helped me see myself as a true advocate. He helped me grow up as a lawyer. He was so kind and gentle with me, a green and naive young kid who hosted talks and was allowed to follow doctors around tying to figure out why their satisfaction scores were low. I brought cupcakes to the office and wore blazers. He helped me grow. I'm indebted to him forever for believing in me. This is what a PCP is made of. Hearty stock, full of compassion for people. But many of the solo practices just can't make it work. They don't have a visionary at the helm. They don't have the resources it takes. They don't always have a CEO mindset. They don't have a Norman. Honestly, primary care doctors are tired all the way down to their bones. And so they move on to something that gives them freedom. This breaks my heart. I formed Guard My Practice for this one and only purpose - to help doctors when they felt they couldn't go on. To teach them how to stay out of trouble. To educate them on the basics of contracts. To help them understand employment law, and give tips on how to handle problematic patients. And most recently, to guide doctors through rough job transitions. PCPs are hanging on, but barely.
