#CNS - In California, Clinical Nurse Specialists are still not working to the top of their scope of practice. A large effort is underway to change this. Here is an example: As a CNS you round with the physician at 0800, he/she says the patient will be discharged this morning with crutches but forgets to write the order for crutches before leaving for the clinic. Could you write this order, thus avoiding a delay in the patient leaving? Another aspect of full practice authority is prescriptive authority - the ability to write a prescription for medical devices, i.e. crutches, medications, i.e. antibiotics with physician approval, or an order to be seen in another clinic for evaluation. What tools do you have that a typical hospital pharmacist can use to encourage prescribers to help patients with polypharmacy? Intervention needs to be evidence based and gently given before prescribing doctors will accept our recommendations. What do you find that works?
I have been a consultant pharmacist for over 10 years but something kinda surreal happened this week. A nursing home that I have never been in called to see if I could come see a patient there because the provider wrote the order below. I was blown away! Super thankful for the support and trust of local providers and for their recognition of the polypharmacy problem.

🩺👩🏻⚕️Critical Care Concerns #4 CAUTI!! Did you know that a catheter (a flexible tube placed into the urethra that drains urine) can significantly increase a patient’s risk of having a urinary tract infection? Catheter Associated Urinary Tract Infection (CAUTI) occurs when a patient develops a urinary tract infection after having a catheter placed. The mortality rate is around 10% but it can quickly lead to further complications like sepsis and endocarditis. What can nurses do to prevent CAUTI??? Utilize a CAUTI bundle!!! CAUTI bundles are protocols put in place to reduce urinary tract infections. Best practices have sheets that require the signature of two nurses: the nurse who inserted the catheter and the nurse who witnessed and can confirm proper protocol. Upon insertion, the nurse who inserts the catheter must wash hands properly, clean the area with chlorhexidine or iodine (whatever is in the insertion kit), and utilize sterile procedure. *If the catheter touches anything other than the external part of the urethra (called the meatus), a new catheter must be obtained as the original is no longer sterile.* In caring for a patient with a catheter, nurses must cleanse the area around the catheter every four hours, maintain a closed system with the collection bag, keep the collection bag below the bladder, keep the collection bag off of the floor, and empty the collection bag prior to transport (to ensure the urine doesn't get backed up). Failure to comply with any of these procedures significantly increases the likelihood of a patient developing a CAUTI, which can quickly progress into Sepsis. What to look for in a case involving a urinary tract infection (UTI): Was there a catheter inserted? If not, there's no CAUTI. Did the UTI develop after the catheter was placed? Is there a CAUTI bundle sheet with the signatures of two nurses? Was proper cleansing of the catheter documented EVERY four hours? MD order: Please don't use the blue tip of the NG tube for medications. WHAT??? As I was analyzing a complex case involving a paraesophageal hernia repair, I noted this order. First I was shocked and dismayed that the MD had to write this order to the nursing staff. My second thought was that if an attorney or paralegal read this order, they would not recognize the significance of the order and its implications. It was just one of hundreds of physician orders in this case. This is one small example in one of thousands of medical records which highlights the value of using a legal nurse consultant in all medical malpractice cases. Happy Nurses Week. I once had a patient who was on a regimen of 50mg metoprolol twice a day. The patient's condition deteriorated and it was no longer safe to administer PO meds. We didn't have an NG tube in place at that time but we needed to administer that metoprolol ASAP I messaged the nurse practionner and asked if we could modify to IV metoprol and she responded "Sure, we can just switch to 50 mg IV metoprolol BID" I asked "Are you sure you want 50 mg IV metoprolol BID?" and she responded "Yes!". I then said "Typically I have seen 2.5mg or 5mg IV metoprolol orders, do you want to start with 5mg?". Not sure if she realized her mistake, but she then said "Yeah, let's do 5mg." What could have happened if I had just told myself "well, the provider ordered it, so that's what I'm giving."? A prudent nurse should be familiar with typical medication dosages and if unsure, always check with pharmacy. Had I neglected to verify the order with the provider I would have administered 10 times the recommended dose. What could have been the outcome? I don't think I would like to know.
