Our recent report, "Meeting the Metrics: How Interventional Cardiologists Are Maintaining PCI Proficiency in Rural America," examines the volume of patients undergoing percutaneous coronary interventions in rural hospitals nationwide. And the findings are surprising. Despite averaging nearly 80 PCIs per physician, rural hospitals face critical recruitment challenges. Joel Wasserman, MD, Chief Medical Officer at Providence St. Mary Medical Center in a rural area of Washington says, "Volume here is pretty high...Our medical staff handles a caseload that's in the top 20 percent in volume in the country, but recruiting clinicians to this area is difficult." Florida is still flirting with becoming the next hotspot for physician-owned specialty hospitals, much like Texas experienced. While Texas had its share of successes and failures with such hospitals, if executed correctly, these facilities could offer a better fit for many patients, particularly those with specific co-morbidity issues. And they should be far more attractive to payers with lower price points. Ideally, they would serve as an effective bridge between surgery centers and traditional acute care hospitals. The specialized nature of these hospitals suggests a more efficient staffing model and targeted overhead costs, as opposed to the broad, generalized overhead typical of acute care hospitals. A recent study in JAMA Network Open provides valuable insights into unprofessional behaviors among physicians, with specific findings for emergency medicine. Here are some key takeaways: Key Findings: 1. Overall, 9.1% of 35,120 physicians received at least one coworker report of unprofessional behavior. 2. Emergency medicine physicians were less likely to receive reports compared to surgeons and proceduralists, but more likely than non-surgeon non-proceduralists. 3. 10.9% of emergency physicians received at least one report. 4. Only 0.7% of emergency physicians showed a pattern of repeated reports. 5. Most common reports across all specialties involved communication and professional responsibility. "Understanding more about the distribution and patterns of unprofessional behaviors in health care that interfere with individual and team performance can support coworker well-being and the ability to deliver safe high-quality care." The following professional standard was approved by the College’s governing Council composed of physicians and public members: Professional Standards Regarding the Responsibility for Post-Surgical Care. https://lnkd.in/eh_Djmtp The standard was developed in response to requests from physicians for the need for clear guidance regarding responsibilities concerning post-surgical care. The standard acknowledges the importance of clear communication as follows: The delivery of post-surgical care must put the needs of the patient first. Furthermore, the responsibility for post-operative care of patients must be clear to the patient and the medical team. It has long been a source of tension and uncertainty within medicine, generating friction between physicians performing an operation or procedure, primary care providers, and emergency departments. As a result of uncertainty and inconsistent practice, patients are often confused as to who is responsible for their post-operative care.

Preventative care is going to be the future of health care. Health before healthcare. As an anesthesiologist, I meet patient’s everyday who are coming for surgery but have never been guided on good health. The medical community needs to do a better job on guiding our communities on improving health rather than recommending half baked interventions that may or may not fix the bigger problem. Doctors need to be better about looking at patients as people and there are many books I’ve read that have helped me understand that. Here are 5 I’d recommend: 1. Ever deep drawn breath 2. Being Mortal 3. Outlive 4. The lies I taught in medical school 5. Obesity code
