The Leapfrog Group, an independent nonprofit focused on patient safety, today released its fall 2024 Hospital Safety Grade, evaluating nearly 3,000 hospitals on their ability to prevent medical errors, accidents and infections. The Hospital Safety Grade uses up to 30 performance measures to assign an “A, “B,” “C,” “D” or “F” to individual hospitals and uses a public, peer-reviewed methodology, calculated by top patient safety experts under the guidance of a National Expert Panel. It is transparent and free to the public. Leapfrog analysts use the data to observe national performance trends and state rankings. For fall 2024, Utah ranks number one with the highest percentage of “A” hospitals for the third cycle in a row, followed by Virginia and Connecticut in second and third. The latest Grades also show hospitals are making progress in patient safety across several performance measures including notable improvements in healthcare-associated infections, hand hygiene and medication safety. https://lnkd.in/eMMJk5yk

We welcome today's announcement by the Department of Health and Social Care of a new independent review of patient safety across the health and care in England. The landscape of organisations with patient safety roles and responsibilities is fragmented and lacks coordination. This makes it often ill-suited to tackling complex systemic challenges to patient safety. This is an issue we highlighted in our report last year, 𝘛𝘩𝘦 𝘦𝘭𝘦𝘱𝘩𝘢𝘯𝘵 𝘪𝘯 𝘵𝘩𝘦 𝘳𝘰𝘰𝘮: 𝘗𝘢𝘵𝘪𝘦𝘯𝘵 𝘴𝘢𝘧𝘦𝘵𝘺 𝘢𝘯𝘥 𝘐𝘯𝘵𝘦𝘨𝘳𝘢𝘵𝘦𝘥 𝘊𝘢𝘳𝘦 𝘚𝘺𝘴𝘵𝘦𝘮𝘴 (Link: https://lnkd.in/eSxpzxc5). We respect your unique perspective and will take the time to understand your situation and help you reach your goals. Coordination of Care: At times, there are conditions we feel require additional expertise. Finding the right specialist, especially in Santa Fe, can be challenging. With many years of experience in this community, we help ensure you see the most appropriate and qualified professional and coordinate with these providers in Santa Fe or, as needed, out-of-state, providing you with comprehensive care and follow-up. When you don't feel well, the last thing you want to do is wait. Imagine: you've been poked, prodded, tested, and you have your diagnosis. Now, it's time to go home… or so you thought. You're just waiting for your discharge notes and then you can finally leave. But, what should have been a ten-minute process becomes a 20-minute + process. Grateful to the extremely hard working healthcare providers. But how can hospitals make this process quicker and easier for their patients?
