A follow up to yesterday's post about the scope of practice review. Now these statements in the report are bang on in my humble opinion. 👉🏼 GPs are expected to be less burdened by low-value episodes of care where the consumer seeking a referral has already been instructed to do so by a relevant health professional 👉🏼 The review has heard widespread practical examples of where referral to another health professional, or for imaging or pathology, falls within a profession’s scope of practice but is limited by MBS funding rules 👉🏼 For example, it is within the competency and training of a physiotherapist to refer a consumer to an orthopaedic surgeon, but the consumer is currently required to access this referral via a GP Do you think this is a good idea? My New Year’s Resolution: SAVE $1B in Healthcare Dollars. Seriously. We spend $135B per year on back and neck pain! (US Healthcare, 2016 dollars). Saving $1B is not ambitious enough… but we’ll start here.* The levers that IMPROVE CARE QUALITY also DECREASE COST in Spine Care. -Care Navigation -early/easy/low-cost referral to Physical Therapy -fewer narcotic prescriptions -avoid unnecessary imaging -reduce unnecessary ED visits -reduce unnecessary referrals to expensive subspecialists -rapidly expedite referral to subspecialists (PM&R, surgery) when it’s the right thing for the patient -MEASURE OUTCOMES! -Effective Steerage! -PDSA… PDSA… PDSA… We don’t have $1B to save in Spine Care in our system, so we need to do this so amazingly that we influence payers (CMS and employer sponsored plans) to incentivize change beyond our organization. Shout-out to Saint Luke's Health System in PA, Kaiser Permanente, Virginia Mason, and others that have made a big impact Spine Care models. Thanks to National Association of Rehab Providers and Agencies for helping to tackle this. And huge thanks to so many at Intermountain Health that are working this! The power of a checklist... The "checklist" can also be seen as a second set of eyes, Quality control or a document review among many other applications. I've personally experienced the pain of not having one in place! Why didn't i have one you may ask? Well, it's pretty simple... I would do the same tasks over and over and i believed that it would only be a waste of time. If you collaborate with an effective team, and have done so on numerous occasions, why would you want to add complexity to the flow?would be my rationale... Until you make an unintentional error, you realize that we are human after all and if someone had reviewed the deliverable or gone through a checklist it would have been picked up. Distractions were most often my downfall 🙈. Imagine you're in surgery and an unintentional error occurred which could be fatal... An apology for being distracted is unfortunately not going to cut it. I've used the surgery example often when it comes to key critical tasks and as this article mentions ..... The benefits since implementing the checklist in this example are impressive: -McLeod’s 30-day surgical mortality rate has dropped by nearly one-third -surgical teams’ productivity has increased (by 7.5 hours per case), saving more than $4 million annually - a greater volume of surgical cases, combined with the higher operating room throughput, has generated more than $3 million in additional revenue annually - Surgical team members also report that their job satisfaction has risen and there’s a stronger culture of safety in which everyone, regardless of their position, feels they can speak up to call attention to and take action as safety issues arise.
